<div>Job description:
The Case Manager will provide care management services to adults with addiction issues, serious emotional disturbances and severe and disabling mental illnesses under the supervision of the Program Supervisor. Case Managers will assess and mutually determine a treatment plan with consumers, schedule and invite team members to treatment team meetings, monitor treatment plan implementation, adjust goals as indicated, and mutually establish criterion for discharge with consumers. Case Manager will also provide illness management and recovery skills groups and other recovery focused groups, as requested. Case management services will be provided to assist adult consumers to gain access to needed medical, emotional, social, educational, vocational, recreational, and other services as requested or needed. Caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis.

Essential Job Functions:<ul><li>
assess and evaluate adults to determine the appropriateness and need for care management services</li><li>
assist the consumer&rsquo;s entry into service and monitor services provided throughout the service system</li><li>
provide coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitor that plan as required by established policy and procedure</li><li>
coordinate treatment team meetings and ensure that treatment plans are outcome based</li><li>
work positively with team members within the agency and with community providers</li><li>
educate consumers about recovery markers and utilize these markers in evaluating progress on the treatment plan</li><li>
follow policies, procedures, and guidelines as they apply to services provided</li><li>
assure that consumers served are screened for and assisted in enrollment in Medicaid, when applicable</li><li>
Ensure that treatment plans are completed within 5 encounters or 21 days, whichever is sooner, that clinical assessments are updated yearly. If clients are being seen for a mental health diagnosis, a diagnosis of SDMI and a LOI need to be completed to receive case management services. If a client is receiving services to address a SUD diagnosis, the SUD diagnosis will be the criteria for entrance to case management or CBPRS services</li><li>
prepare reports, applications, and supporting documentation on the established timelines</li><li>
attend workshops, conferences, meetings, etc. as agreed upon with the supervisor and be prepared to present learning to other Service Coordinators and employees as requested</li><li>
meet with supervisor bi-weekly or as necessary to discuss individual consumer cases and services provided</li><li>
participate in training required</li><li>
caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis</li><li>
maintain assigned consumer caseload as determined by the agency</li><li>
travel within the geographical area of the assigned caseload to provide needed services</li><li>
provide the bulk of services out in the community rather than in the office</li><li>
provide transport to as requested</li><li>
perform other duties as assigned</li></ul>
Community Based Psychiatric Rehabilitation<ul><li>
Community Based Psychiatric Rehabilitation, (CBPRS) will be provided by Case Managers and will be documented separate from case management, with an individual superbill.</li><li>
CBPRS is intensive behavior management and stabilization services in home, workplace, or community settings, for a specified period, in which the problem or issue impeding recovery or full functioning is defined and treated.</li><li>
The purpose of CBPRS is to reduce disability and restore functioning, and help individuals return to natural settings and activities that are part of a socially integrated life.</li><li>
Through CBPRS, a behavioral aide supports the member by augmenting life, behavioral, and social skills training needed to reach the member&rsquo;s identified treatment goals and restore member functioning in the community.</li><li>
During skills training, the behavioral aide clearly describes the skill and expectations of the member&rsquo;s behavior, models the skill, and engages the member in practice of the skill, and provides feedback on skill performance. Restoring these skills helps prevent relapse and strengthens goal attainment.</li><li>
These aides may consult with family members or other key individuals who are part of a member&rsquo;s treatment team to determine how to help the member be more successful in meeting treatment goal.</li><li>
CBPRS may be reimbursed up to maximum of 2 hours of group CBPRS services and 2 hours of individual CBPRS services in a 24-hour period, unless granted an exception by the department.</li><li>
Group CBPRS may include up to 8 adults in the group per one staff. (6) Face-to-face service delivery of CBPRS services is preferred. Telehealth may be substituted if clinically indicated or if the member does not have access to face-to-face services.</li><li>
Documentation must include reason(s) for telehealth delivery of CBPRS services including attempts to identify local supports if reason(s) for telehealth provision of services is related to access issues.</li></ul>
Attendance Requirements:
Case Managers and CBPRS staff will be expected to have flexible hours that meet consumer scheduling needs. The scheduling of staff meetings and training meetings may necessitate variations in schedules. Permanent changes in an employee&rsquo;s work schedule will be provided to the employee in writing with an explanation why the change was made. Scheduling is the responsibility of the employer and will be based on the needs of the people served by the agency.
Minimum Qualifications: Bachelor's Degree in Social work, or related field helping people.
