Personal Care Assistant (PCA) Key Responsibilities</h3><ul><li>
Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility</li><li>
Provide emotional support and promote resident comfort and independence</li><li>
Observe and report changes in residents&rsquo; condition to nursing staff</li><li>
Maintain a clean, safe, and respectful care environment</li><li>
Follow infection control, safety, and confidentiality guidelines</li><li>
Work collaboratively with nursing and interdisciplinary team members</li></ul><h3 class="jobSectionHeader">
Personal Care Assistant (PCA) Qualifications</h3><ul><li>
Previous PCA, CNA, LNA, or caregiving experience preferred (training available)</li><li>

<div>Physical Therapist

Job Summary:

The Physical Therapist is responsible for the development of an individualized therapy program consisting of physical assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. 


Duties/Responsibilities:
<ul><li>Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients&rsquo; functional needs and adjusts care plans to ensure optimal patient outcomes.</li></ul>
<ul><li>Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.</li></ul>
<ul><li>Conducts therapy practice within defined standards of care.</li></ul>
<ul><li>Directs the assistance of support staff to achieve patients&rsquo; functional goals.</li></ul>
<ul><li>Communicates effectively with other professional and support staff to achieve positive patient outcomes.</li></ul>
<ul><li>Practices facility and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.</li></ul>
<ul><li>Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.</li></ul>
<ul><li>Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.</li></ul>

Required Skills/Abilities:
<ul><li>Knowledge and skills to care for patient needs of the SNF population</li></ul>
<ul><li>Must possess problem solving skills to successfully perform job</li></ul>
<ul><li>Excellent verbal and written communication skills.</li></ul>
<ul><li>Excellent organizational skills and attention to detail.</li></ul>
<ul><li>Strong customer service orientation.</li></ul>
<ul><li>Must be able to communicate effectively and professionally with other employees, patients, and family members</li></ul>
<ul><li>Ability to travel as needed to perform job</li></ul> 


Education and Experience:
<ul><li>Graduate of an accredited Bachelors, Masters, or Doctorate in Physical Therapy.</li></ul>
<ul><li>Proficiency with all applicable state, federal, and professional standards</li></ul>
<ul><li>Current license from the state Board of Physical Therapy Examiners.</li></ul>

Physical Requirements:
<ul><li>Duties require extensive standing and walking.</li></ul>
<ul><li>Requires lifting, positioning, pushing, and/or transferring patients.</li></ul>
<ul><li>Duties require frequent reaching, stooping, bending, kneeling, and crouching.</li></ul></div>
