Patient Care for CNA Home Hospice Care:
<ul><li>
Travel to where Patient lives and calls home.</li><li>
Provides care and services to patients according to the care plan or as approved by an RN.</li><li>
Assists with other activities of daily living as directed by an RN.</li><li>
Offers support and encouragement to patients and families.</li><li>
Functions within acceptable time frames for daily patient visits.</li><li>
Clearly communicates information regarding changes in the patient&rsquo;s mental/physical condition, complaints, or special requests to the Agency nurse, and ECF unit manager if applicable.</li><li>
Demonstrates good judgment in knowing what information needs to be communicated.</li><li>
Handles communication in a timely manner.</li><li>
Maintains documentation of patient care provided on both Agency forms, and the ECF record if applicable.</li><li>
Uses appropriate Mobile Care documentation module or paper forms.</li><li>
Records information accurately, thoroughly, and in a timely manner</li><li>
Committed to Agency resolution against fraud and abuse.</li><li>
Knowledgeable in hospice philosophy and state and federal rules and regulations.</li><li>
Respects confidentiality of patients/families and other Agency employees.</li><li>
Attends educational meetings/conferences for professional growth as required.</li><li>
Follows directions and policies that allow for an organizational commitment to working under time pressures without sacrificing quality.
</li></ul>Successful Capabilities of CNA Home Hospice Care:
<ul><li>
Must be able to read and write English proficiently.</li><li>
Must be state tested nursing assistant in good standing</li><li>
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.</li></ul>

Conditions of Employment for CNA Home Hospice Care:
<ul><li>
Prior training and experience as a Home Health Aide with Indiana certification as a Nurse Aide.</li><li>
Must pass Drug Screen and Background check.</li><li>
Compliance to Annual Flu Shot and Covid Vaccination Policy or ability to provide exemption documentation</li><li>
Pass TB test</li><li>
Provide an active auto insurance policy as a licensed driver in the State of Indiana with the proper level of coverage as directed</li><li>
Provide an active driver&rsquo;s license</li><li>
Ability to provide proof of eligibility to work in the United States.</li><li>
High level of integrity, ethics and professionalism.</li></ul>
