Primary Care Physician

The Patient Services Coordinator is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management<ul><li>
Manages schedules for all patients. Edits schedule for agents calling in sick, ensuring patients are reassigned timely. Updates agent unavailability in worker console.</li><li>
Initiates infection control forms as needed, sends the HRD the completed &ldquo;Employee Infection Report&rdquo; to upload in the worker console.</li><li>
Serves as back up during the lunch hour and other busy times including receiving calls from the field staff and assisting with weekly case conferences. Refers clinical questions to Branch Director as necessary.</li><li>
Maintains the client hospitalization log, including entering coordination notes, and sending electronic log to all office, field, and sales staff.</li><li>
Completes requested schedule as task appears on the action screen. Ensures staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.</li><li>
Completes requested schedules for all add-ons and applicable orders:<ul><li>
Schedules discharge visit / OASIS Collection or recert visit following case conference when task appears on action screen.</li><li>
Schedules TIF OASIS collection visits and deletes remaining schedule.</li><li>
Reschedules declined or missed (if appropriate) visits.</li><li>
Processes reassigned and rescheduled visits.</li><li>
Ensures supervisory visits are scheduled.</li></ul></li><li>
Runs all scheduling reports including Agent Summary Report and Missed Visits Done on Paper Report.</li><li>
Prepares weekly Agent Schedules. Performs initial review of weekly schedule for productivity / geographic issues and forwards schedule to Branch Director for approval prior to distribution to staff.</li><li>
Verifies visit paper notes in scheduling console as needed.</li><li>
Assists with internal transfer of patients between branch offices.</li><li>
If clinical, receives lab reports and assesses for normality, fax a copy of lab to doctor, make a copy for the Case Manager, and route to Medical Records Department. Initiate Employee / Patient Infection Reports as necessary.</li><li>

Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care (POC).</li><li>

Business Intelligence Engineer

Essential Functions:
Administrative<ul><li>
Prepare patient assessment packets, including admission, recertification, and post hospital packets.</li><li>
Process signed and unsigned orders and 485s.</li><li>
Follow up on the Order Tracking Report weekly and according to Order procedure. Reprocess unsigned orders at end of episode as task appears on action screen.</li><li>
Date stamp all received documents, except SOC paperwork and paper visit notes, to be scanned into the client chart. Scanning should be completed within 24 hours of stamp date.</li><li>
Fax the discharge / transfer summary / episode detail summary and Case Conference / 60 Day summary to the physician as task appears on the action screen.</li><li>
Copy documentation and assist with ADR preparation.</li><li>
Update HCHB and computer programs as appropriate.</li><li>
Complete End of Period claim workflow as part of Billing Specialist responsibilities. This includes confirming all orders are signed appropriately and coordinating with the administrative team at the branch to ensure other billing requirements are satisfied to release claims timely.</li><li>
Adhere to and participate in Agency&rsquo;s mandatory HIPAA / Privacy Program and Employee Compliance Program.</li><li>
Read and adhere to all Agency Policies and Procedures and follow Employee Handbook Guidelines.</li><li>
Managing all incoming telephone calls in a professional manner including:<ul><li>
Screen and route calls to the appropriate agency staff</li></ul></li><li>
Maintenance and administration of the phone system including:<ul><li>
Adding / revising voice mail boxes</li><li>
Managing accurate employee phone roster</li></ul></li><li>
Process incoming and outgoing mail.</li><li>
Manage and order office supplies (including agency specific forms) as needed.</li><li>
Manage, order, and distribute medical supplies as needed.</li><li>
Assist BD with EOE billing tasks.</li><li>
Manage and process all accounts payable including routing to appropriate corporate department as needed.</li></ul><h1 class="jobSectionHeader">

</h1>Knowledge/Skills/Abilities:
<ul><li>Must understand the issues related to the delivery of home health and hospice services and be able to problem solve effectively.</li><li>
Must possess knowledge of Medicare guidelines governing home health and hospice agencies.</li><li>
Must have exceptional customer service skills and be able to communicate well with referral sources, patients, family members, and agency employees.</li><li>
Must be organized, detail-oriented, and possess effective communication skills both orally and in writing.</li><li>
The ability to communicate with a diversity of individuals is required.</li></ul>
Qualifications:
Education:
<ul><li>Must have a high school diploma or equivalent, typing, clerical skills, and be competent with computers.</li></ul>
Experience:
<ul><li>Must possess a minimum of two years&rsquo; experience in the health care industry and one year experience in home health.</li></ul>

The Specialty Operations Pharmacy Technician 2 supports Pharmacists in Specialty Rx medication preparation by dispensing Specialty Rx medications. The Specialty Operations Pharmacy Technician 2 performs varied activities and moderately complex administrative/operational/customer support assignments. Performs computations. Typically works on semi-routine assignments.
The Specialty Operations Pharmacy Technician 2 May contact physicians for prescriptions clarification and may enter prescription orders into the system. Also supports Pharmacists and Patients by ensuring adherence to Specialty Rx specific operational processes and programs developed to optimize health outcomes which includes effective communication with patients and providers. Decisions are typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, andworks under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.<h1 class="jobSectionHeader">

Schedule: Monday-Friday, 8 hour shift between the times of 8 AM -11PM EST
