The Behavioral Health Case Manager is responsible for providing evidence-based psychological and behavioral health services within post-acute and long-term care facilities. This role delivers direct clinical care, conducts psychological and behavioral assessments, and collaborates with interdisciplinary teams to support patient well-being, reduce hospitalizations, and enhance facility outcomes.</p><p>
The position requires an independently licensed clinician who can provide psychotherapy, behavioral interventions, crisis response, and care coordination while adhering to all applicable state and federal regulations. The Behavioral Health Case Manager integrates mental health services into the facility&rsquo;s broader clinical environment and supports value-based care by addressing emotional, cognitive, and behavioral needs in medically complex populations.</p><p>
This role requires strong clinical judgment, cultural competence, proficiency with EHR systems, adherence to compliance and documentation standards, and the ability to communicate effectively with facility staff, patients, families, and internal teams.</p><p></p><h2 class="jobSectionHeader"><b><br>
Key Responsibilities</b></h2><h3 class="jobSectionHeader"><b>
Clinical Care</b></h3><ul><li>
Provide psychotherapy using evidence-based interventions appropriate for post-acute and long-term care residents.</li><li>
Deliver behavior therapy and supportive interventions to address mood, cognitive, and behavioral symptoms.</li><li>
Conduct psychological and behavioral assessments to support diagnosis, care planning, and treatment goals.</li></ul><h3 class="jobSectionHeader"><b>
Care Coordination &amp; Collaboration</b></h3><ul><li>
Collaborate with physicians, nursing, therapy, social services, and interdisciplinary team members to support integrated behavioral health care.</li><li>
Participate in care planning meetings as needed and communicate treatment recommendations clearly to facility teams.</li><li>
Coordinate with internal PHA providers to support continuity of psychiatric and psychological care.<br>
</li></ul><h3 class="jobSectionHeader"><b>Documentation &amp; Compliance</b></h3><ul><li>
Complete accurate, timely documentation within the EHR in accordance with PHA policies, billing requirements, and regulatory standards.</li><li>
Follow all AHCA, DCF, CMS, and Florida practice guidelines relevant to psychological and behavioral health care.</li>
Maintain ethical practice standards, confidentiality, and HIPAA compliance at all times.<br>

Crisis Intervention</b></h3><ul><li>
Provide crisis assessment and intervention, including managing acute changes in mental status, suicidality, aggression, or behavioral disturbances.</li>
Follow appropriate procedures for risk management, safety planning, and Baker Act protocols when indicated.<br>

Education, Consultation &amp; Facility Support</b></h3><ul><li>
Provide consultation and guidance to facility staff to enhance understanding of mental health conditions, behavioral strategies, and patient care approaches.</li>

Maintain required licensure, continuing education credits, and clinical competencies.<br>

Qualifications</b></p><h3 class="jobSectionHeader"><b>
Education</b></h3><ul><li>
Doctoral or master&rsquo;s degree in psychology, counseling, , or social work from an accredited institution.</li></ul><h3 class="jobSectionHeader"><b>
Licensure (Required)</b></h3><p>
Active Florida independent license as one of the following:</p><ul><li>
Licensed Clinical Social Worker (LCSW)</li><li>
Licensed Mental Health Counselor (LMHC)</li></ul><h3 class="jobSectionHeader"><b>
Experience</b></h3><ul><li>
Minimum of two years of clinical experience in behavioral health.</li><li>
Experience working with geriatric, medically complex, or post-acute care populations preferred.</li><li>
Experience with behavioral or cognitive interventions, crisis response, and interdisciplinary collaboration preferred.</li></ul><h3 class="jobSectionHeader"><b>
Knowledge &amp; Skills</b></h3><ul><li>
Proficiency in evidence-based therapeutic approaches (CBT, supportive therapy, solution-focused therapy, etc.).</li><li>
Strong clinical assessment, diagnostic, and problem-solving skills.</li><li>
Understanding of interdisciplinary care models within skilled nursing and long-term care environments.</li><li>
Familiarity with value-based care principles and outcome-driven clinical practices.</li><li>
Proficiency with EHR documentation and digital communication tools.</li><li>
Strong interpersonal, communication, and organizational skills.</li><li>
Demonstrated cultural competence and ability to work effectively with diverse patient and facility populations.</li></ul><h2 class="jobSectionHeader"><b>
Work Environment &amp; Schedule</b></h2><ul><li>
On-site conducting screenings across assigned post-acute and long-term care facilities. 50% travel to buildings / 50% Working from home-office</li><li>
Monday&ndash;Friday with some flexibility based on facility and patient needs. (8am-5pm)</li><li>
Credentialing support provided for payer enrollment.</li></ul></div><p></p>
