Central Valley PACE

<div>Supports mission, vision, and values of Golden Valley Health Centers Central Valley PACE Center by acting as the PCA and delivering patient care activities and tasks as assigned to assist the transportation and nurse staff.

Location: Central Valley PACE Center in Modesto, Ca.

Schedule: Monday - Friday. This position requires availability between the hours of 7:30am-4:30pm.

Compensation: $24.31 - $25.52 an hour

Duties and Responsibilities
<ul>
<li>Communicates patient care needs to the LVN and supervising RN.</li>
<li>Provides direct personal care to participants as directed or assigned.</li>
<li>Assists in training new hire PCA&rsquo;s and other staff as needed or directed by management.</li>
<li>Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants. Duties will include showering, toileting, laundry and feeding assistance and companionship services to and from medical appointments.</li>
<li>Provides assistance with ADLs and IADLS, transfers, companionship to doctor&rsquo;s appointments, etc.</li>
<li>Provides support and assistance with meal and snack service in the Day Center.</li>
<li>Assists in maintaining patient areas in a clean and orderly manner. Cleans and organizes program areas including cleaning of equipment and shower areas.</li>
<li>Communicates effectively with participants, families, staff, other departments and agencies.</li>
<li>Works effectively in a team setting.</li>
<li>Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.</li>
<li>Participates in teaching/learning process with others.</li>
<li>Follows guest relations policies and procedures.</li>
<li>Abides by all established policies, rules and regulations.</li>
<li>Participates as a member of the interdisciplinary team (IDT).</li>
<li>Attends staff meetings and takes part in participant care planning.</li>
<li>Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.</li>
<li>Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.</li>
<li>Performs patient care activities appropriately, reporting any circumstances or change in patient&rsquo;s condition.</li>
<li>Performs clerical and administrative work as required including making and answering phone calls, assisting with the implementation of closure policies as per instructed by executive leadership . Entering participant data in EHR system and assisting covering reception.</li>
<li>Assist and have ability to escort participants that require assistance in their care plan.</li>
<li>Transport participants on an as needed basis.</li>
<li>Other projects and duties as assigned.</li>
</ul>
Physical Demands
<ul>
<li>Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.</li>
<li>Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.</li>
<li>Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.</li>
<li>Must have high manual dexterity.</li>
<li>Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.</li>
</ul>
Work Environment
<ul>
<li>Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.</li>
<li>Subject to unpleasant odors.</li>
</ul>
The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Minimum Qualifications
<ul>
<li>Valid California Driver&rsquo;s License, acceptable driving record and vehicle insurance.</li>
<li>Must be able to treat people of all ages including geriatrics.</li>
<li>Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.</li>
</ul>
Education/Experience
<ul>
<li>High School Diploma or equivalent required.</li>
<li>Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.</li>
<li>Current BLS CPR Card certified by the American Heart Association</li>
<li>Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.</li>
<li>Prior transportation experience, or ability to pass internal competencies within six (6) months of hire.</li></ul></div>

PACE Personal Care Assistant II Transportation (Central Valley PACE - Modesto)

<div>Under the supervision of the Transportation Coordinator, the Medical Transporter is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.

Location: Stoddard Rd Modesto, CA 95356 
Schedule: Monday - Friday. This position requires availability between the hours of 6:00am-6:00pm. 
Compensation: $21.79 - $22.88 an hour.

Duties and Responsibilities
<ul><li>Effectively communicates with participants, families, staff and other agencies and departments.</li>
<li>Drives a specially equipped van to deliver physically limited persons to varied destinations.</li>
<li>Assures wheelchair lifts and tie down equipment are properly operated and participants are delivered to appointments in a timely manner.</li>
<li>Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes.</li>
<li>Assists passengers on and off the van in a safe and courteous manner. Will work with medical equipment and manage other emergencies that may arise.</li>
<li>Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.</li>
<li>Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.</li>
<li>Inspects van and maintains it in a clean, orderly and safe condition.</li>
<li>The driver must exhibit exceptional customer service to patients, families, case managers, facilities, physicians, and nurses.</li>
<li>Completes driver log each time the vehicle is used. Keep an accurate record of all facility transports, mileage and times. Provides facility administrator with completed logs.</li>
<li>Inspects the company vehicle monthly using the Vehicle Maintenance Log.</li>
<li>Reports all maintenance or vehicle needs to the facility administrator timely and follow up as appropriate.</li>
<li>Assists with maintaining all maintenance records and other records regarding the company vehicle.</li>

<li>Completes documentation as required. Coordination of 24 hour care delivery.</li>
<li>Other projects and duties as assigned.</li>

<li>Minimum six (6) months experience transporting individuals with special mobility needs required.</li>
<li>Current BLS CPR Card certified by the American Heart Association</li>
<li>Certified Nurse Assistant preferred.</li>
<li>Prior personal care assistance experience, or ability to pass internal competencies, preferred.</li></ul></div>
