Job Profile Summary 
Role:
 
The RN, Care Coordinator is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.
Qualifications:<ul><li>
Current license as RN in the state where the employee will be working</li><li>
Minimum of one (1) year nursing experience; hospice or hospital experience preferred</li><li>
Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system</li><li>
Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred</li><li>
Mobile Driver - Valid driver&rsquo;s license and automobile insurance per Company policy</li><li>
Reliable transportation to meet patients&rsquo; needs</li><li>
Excellent critical decision making and problem-solving skills</li><li>

Completes clinical admissions for all Company services including, but not limited to:<ul><li>
Explains services to potential patients/families and addresses questions regarding patient fears, physical limitations, while putting the patient/family at ease; presents services in an empathetic and compassionate manner</li><li>
Identifies admission objections, concerns or other entry barriers and provides responses</li><li>
Educates patient/family on admission requirements and addresses immediate needs</li><li>
Completes informed consents and initial assessment according to regulations and Company policy</li><li>
Provides information to Physicians and initiates Plan of Care to address patient&rsquo;s immediate needs</li><li>
Initiates skilled nursing interventions to prevent complications, alleviate symptoms and maximize physical and emotional comfort</li><li>
Obtains Physician orders</li><li>
Completes documentation per Company policy</li></ul></li></ul><ul><li>
Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.</li><li>
Communicates frequently with Admissions to ensure timely follow-up. When assignment is complete, informs Admissions and prepares for next assignment.</li><li>
Provides all necessary clinical communication timely using SBAR.</li><li>
Discusses any potential needs with after-hours staff.</li><li>
Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations.</li><li>
Obtains medical record information for the referral.</li><li>
Develops strong relationships with case managers, physicians, etc. at facilities.</li><li>
Works with Professional Relations Representatives (PRRs) to provide education on all available services.</li><li>
Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge.</li></ul>

Job Responsibilities:<ul><li>
Makes after-hours visits to patients in their home, assisted living facility, nursing home, or hospital, anywhere within the assigned affiliate(s) territory. Provides compassionate customer service to patients and families.</li><li>
Assesses patient/family and identifies immediate needs and appropriate nursing interventions by phone and/or during visit.</li><li>
Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.</li><li>
Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.</li><li>
Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.</li><li>
Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices.</li><li>
Reports changes in the patient&rsquo;s condition to appropriate members of the IDG or other health professionals.</li><li>
Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.</li><li>
Completes documentation per policy and procedure.</li><li>
During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.</li><li>
Perform other duties as assigned.</li></ul>
Compensation Pay Range:
$67,976.53 - $106,213.33

Hybrid position - remote 4 days a week and in office 1 day a week 
 
Role: 
The Accounts Payable Representative is responsible for ensuring accurate payments to vendors/providers which includes processing invoices/claims, conducting research, obtaining approval, recordkeeping, and maintaining vendor/provider relationships.<div>
Qualifications:<ul><li>
Minimum of one (1) year accounting experience to include General Ledger, AP, A/R or Financial Reporting</li><li>
For Patient Payables - preferred two (2) years experience in medical claims processing, medical biller or coder.</li><li>
Computer experience to include Microsoft (Outlook, Excel, Word) and Windows Explorer</li><li>
Team player and self-starter who is accurate and detail-oriented</li><li>
Professional attitude</li><li>
Highly organized, with the ability to effectively manage many tasks simultaneously</li><li>

Job Responsibilities:
 Overhead Payables:<ul><li>
Process invoices/check requests using automated AP system</li><li>
Enters vouchers into AP system and reviews for accuracy. For employee expense reports, make sure all receipts are attached and adheres to policy.</li><li>
Assigns correct affiliate/GL account coding to each voucher.</li><li>
Assigns approver and submits for approval.</li><li>
Adds any voucher that requires special attention to the special handling log.</li><li>
For new vendors, obtains a copy of the vendors&rsquo; W-9 as well as ACH information for proper set-up into the system.</li><li>
Communicates with vendors and teams with regards to problems/questions concerning invoices.</li><li>
Updates all required logs.</li><li>
Reviews system queues/reports to ensure all invoices are being paid in a timely manner.</li><li>
Assists with annual 1099 process.</li></ul>
 Patient Payables:<ul><li>
Efficiently and accurately analyzes claim submissions to determine whether the claim should be accepted, rejected, approved or denied for payment based on the legally binding agreement and/or fee schedules and the patient conditions.</li><li>
Resolves pended claims, secondary review claims and prior approval requests.</li><li>
Reviews and addresses provider inquiries regarding claim adjudication, including incoming phone calls, correspondence, and appeals.</li><li>
Research requests for overpayment/underpayment efficiently and accurately, submit data and resolution to the supervisor for final determination.</li><li>
Maintains a thorough knowledge of third-party billing and reimbursement requirements.</li><li>
Ensure accurate record keeping of all vendors, including TIN, NPI, Physical Address, Remit Address and Vendor Contact person&rsquo;s email address and phone number.</li></ul>
Compensation Pay Range:
$17.89 - $25.76
