The RN Day Triage provides telephonic and ad hoc support to patients and families. The RN, Day Triage will establish and maintain positive relationships with team members, patients/families and referral resources.
Qualifications:<ul><li>
Current Affiliate state or multi-state license to practice as an RN, and has the ability to apply for/obtain multi-state RN license within 30 days of hire, if not already a holder of a multi-state license.</li><li>
Minimum of one (1) year nursing experience; hospice or hospital experience preferred</li><li>
Active BLS for healthcare professionals from the American Heart Association or Red Cross.</li><li>
Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system</li><li>

Job Responsibilities:<ul><li>
Triages phone calls and ensures that callers&rsquo; needs are met efficiently using excellent customer service skills.</li><li>
Provides and manages direct care to patients and families incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.</li><li>
Responsible for the timely, courteous, and effective handling of clinical inquiries related to Chapters Health System&rsquo;s services. Positively promotes and clearly explains benefits of Chapters Health System&rsquo;s services. Clients include current and prospective patients, friends, or family members, Powers of Attorney, care providers, physicians, nursing staff, nursing homes and Assisted Living Facilities (ALFs), hospital staff, and others, as appropriate.</li><li>
Gathers necessary information and required documentation from appropriate sources to ensure complete, current, and accurate documentation of patient information and data.</li><li>
Accurately enters data into information tracking systems, including entering and tracking patient referral file in electronic medical record (EMR).</li><li>
Provides reassurance to patients and families. Assists in finding solutions to their questions and/or recognizes the need for an in person visit. Coordinates or completes in person visit when needed/or requested.</li><li>
Assists the patient and family to achieve the desired comfort levels.</li><li>
Utilizes appropriate support/expert resources or personnel to resolve complex or difficult situations.</li><li>
Documents patient/family contact information in the EMR and communicates with the Interdisciplinary Group (IDG).</li><li>
Triage RN's who experience technical issues not caused by the company equipment or network (i.e. offsite power outage, personal internet outage) are required to report the situation to leadership immediately, and have 30 minutes to resolve the technical issue. If the issue cannot be resolved in 30 minutes, RN may be required to report to a IPU or Resource Center based on leadership direction.</li><li>
During times of emergencies (i.e. Hurricanes, etc.), the Triage RN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency.</li><li>

Compensation Pay Range:
$61,860.66 - $96,657.28

The RN, Care Coordinator is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.

Provides all necessary clinical communication timely using SBAR.</li><li>
Discusses any potential needs with after-hours staff.</li><li>
Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations.</li><li>
Obtains medical record information for the referral.</li><li>
Develops strong relationships with case managers, physicians, etc. at facilities.</li><li>
Works with Professional Relations Representatives (PRRs) to provide education on all available services.</li><li>
Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge.</li></ul>

Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.</li><li>
Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.</li><li>
Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care.</li><li>
Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices.</li><li>

Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support.</li><li>

The Accounts Payable Representative is responsible for ensuring accurate payments to vendors/providers which includes processing invoices/claims, conducting research, obtaining approval, recordkeeping, and maintaining vendor/provider relationships.<div>

Minimum of one (1) year accounting experience to include General Ledger, AP, A/R or Financial Reporting</li><li>
For Patient Payables - preferred two (2) years experience in medical claims processing, medical biller or coder.</li><li>

 Overhead Payables:<ul><li>
Process invoices/check requests using automated AP system</li><li>
Enters vouchers into AP system and reviews for accuracy. For employee expense reports, make sure all receipts are attached and adheres to policy.</li><li>
Assigns correct affiliate/GL account coding to each voucher.</li><li>
Assigns approver and submits for approval.</li><li>
Adds any voucher that requires special attention to the special handling log.</li><li>
For new vendors, obtains a copy of the vendors&rsquo; W-9 as well as ACH information for proper set-up into the system.</li><li>
Communicates with vendors and teams with regards to problems/questions concerning invoices.</li><li>
Updates all required logs.</li><li>
Reviews system queues/reports to ensure all invoices are being paid in a timely manner.</li><li>
Assists with annual 1099 process.</li></ul>
 Patient Payables:<ul><li>
Efficiently and accurately analyzes claim submissions to determine whether the claim should be accepted, rejected, approved or denied for payment based on the legally binding agreement and/or fee schedules and the patient conditions.</li><li>
Resolves pended claims, secondary review claims and prior approval requests.</li><li>
Reviews and addresses provider inquiries regarding claim adjudication, including incoming phone calls, correspondence, and appeals.</li><li>
Research requests for overpayment/underpayment efficiently and accurately, submit data and resolution to the supervisor for final determination.</li><li>
Maintains a thorough knowledge of third-party billing and reimbursement requirements.</li><li>
Ensure accurate record keeping of all vendors, including TIN, NPI, Physical Address, Remit Address and Vendor Contact person&rsquo;s email address and phone number.</li></ul>
