<div>Sign-on Bonus: $2,000<ul><li>
Single point of accountability and continuity of care for adults and/or children receiving behavioral health services.
</li><li>Provides assessment of needs.
</li><li>Develops service plan based on identified needs and monitors implementation.</li><li>
Links individuals to appropriate community service agencies.
</li><li>Provides support to individuals in accordance with 65E-15 FS and Medicaid regulations.
</li><li>Responsible for ensuring individuals are receiving appropriate services, based on identified needs.</li><li>
Coordinates effective transition/discharge planning.</li><li>
Coordinates with caregivers in reference to individual needs, as appropriate.
</li><li>Coordinates multiple complex priority crises.
</li><li>Manages patient placements and placement options.
</li><li>Manages a maximum caseload of 40 individuals. Children count as two (2) on the caseload.
</li><li>Provides services in individual/group homes and other community locations.
</li><li>Documents all interactions with and on behalf of the individual served in the electronic medical record (EMR).</li><li>
Transports individuals served, when needed, in agency or personal vehicle.

Bachelor&rsquo;s degree from an accredited university or college with major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field, and</li><li>
Preference of one (1) year of full-time or equivalent experience working with individuals with serious mental illness.</li></ul>
