The Risk Adjustment Coder works in a collaborative effort directly with physicians and their office staff and other support departments to review medical records and other clinical documentation to identify appropriate risk adjustment codes and quality gap closure opportunities. 
A major focus of the position is to collect and review documents to support the organization&rsquo;s quality and risk adjustment initiatives, which results in improving quality of care.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:<ul><li>
Ensures compliance with all applicable Federal, State and/or County laws and regulations related to coding and documentation guidelines for Risk Adjustment Reviews of medical records, patient medical history and physical exams, physician orders, progress notes, consultation reports, diagnostic reports, operative and pathology reports, and discharge summaries to verify whether:<ul><li>
The diagnosis codes are supported by the documentation and ensure with ICD-10-CM Guidelines for Coding and Reporting.</li><li>
The diagnosis codes for each chronic or major medical condition have been captured correctly.</li></ul></li><li>
Any diagnosis code that is unsubstantiated by the record should be queried to provider and assess to accuracy.</li><li>
Reviews for clinical indicators and query providers to capture the severity of illness of the patient.</li><li>
Conducts medical charts to identifying opportunities for improving individual member risk adjustment score accuracy.</li><li>
Provides feedback to internal clients on:<ul><li>
Examples of documentation and physician self-coding that do not meet quality standards.</li><li>
Examples of missed operations missed opportunities.</li><li>
Examples of clinical that ensure quality and timely care of our members as well as correct reimbursement.</li><li>
Identifies clinical coding and documentation trends and training needs to improve the quality of documentation to reflect our patients&rsquo; health data.</li></ul></li><li>
Attends all meetings as required.</li><li>
Other duties as assigned and modified at manager&rsquo;s discretion.</li></ul>
KNOWLEDGE, SKILLS &amp; ABILITIES:<ul><li>
Advanced understanding of medical terminology, body systems/anatomy, physiology and concepts of disease processes.</li><li>
Demonstrated ability to utilize a variety of electronic medical records systems.</li><li>
Ability to manage significant work load, and to work efficiently under pressure meeting established deadlines with minimal supervision.</li><li>
Strong time management skills.</li><li>
Excellent written and oral communication for representation of clear and concise results.</li><li>
Strong follow-up skills &amp; organizational skills required.</li><li>
Must possess high degree of accuracy, efficiency and dependability.</li><li>
Candidate will start in office, but could potentially work from home after quality and production levels exceed targets. Would need to be comfortable coming to the office on a weekly basis and as established by management.</li><li>

EDUCATION AND EXPERIENCE CRITERIA:<ul><li>
High School Diploma or GED required.</li><li>
Coding Certificate required. APPC or AHIMA coding certified preferred.</li><li>
CRC (certified risk coder) is required, or minimum of 3-5 years&rsquo; experience in risk adjusting coding in lieu of certificate.</li><li>
Two (2) + years&rsquo; experience in a primary care environment is required.</li><li>
Strong knowledge of Microsoft Office Suite (Excel-basic mathematical formulas, charts, tables).</li><li>
Strong medical coding and third party operating procedures and practices.</li><li>
Knowledge of CPT/ICD-9 &amp; 10 &amp; Medical Terminology.</li></ul>
PAY RANGE:
$24.0 - $34.25 Hourly

Risk Adjustment Coder-1

The Care Promoter 1 is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, Care Promoters perform clinical duties under direct physician supervision in accordance with state medical practice acts. The Care Promoter 1 is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:<ul><li>

ESSENTIAL DUTIES AND RESPONSIBILITIES:<ul><li>
Coordinates and processes patient referrals to completion with precision, detail and accuracy.</li></ul>
Definition of completion:<ul><li>
Prioritizes HPP patients in Primary Care Physicians panel, stats, expedites and orders over 5 days.</li><li>
Orders have been approved (when needed).</li><li>
Schedules patient (Preferred Providers List of Specialist) and notifies them of appointment information, including, date, time, location, etc.</li><li>
Uses Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR for authorization processing).</li><li>
Completes orders with proper documentation on where patient is scheduled and how patient was notified.</li><li>
Referrals have been sent to specialist office &amp; confirmed receipt.</li><li>
Prepares and actively participates during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide. Communicates effectively the physicians/clinicians needs or outstanding items regarding to patients.</li><li>
Enters all Inpatient and Outpatient elective procedures in HITS tool.</li><li>
Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.</li><li>
Participates in Super Huddle and provides updates on high priority patients referrals.</li><li>
Addresses referral based phone calls for Primary Care Physicians panel.</li><li>
Completes and addresses phone messages within 24 hours of call.</li><li>

Retrieves consultation notes from the consult tracking tool.</li><li>

Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.</li><li>
Participates in Super Huddle and provides updates on high priority patients referrals.</li><li>
Addresses referral based phone calls for Primary Care Physicians panel.</li><li>
Completes and addresses phone messages within 24 hours of call.</li><li>
Checks out patients based on their assigned physician/clinician. (Note: If assigned Care Coordinator is unavailable at the time of check out, a colleague shall assist patient. This process does not apply to Care Specialist)</li><li>
Retrieves consultation notes from the consult tracking tool.</li><li>
Follows up on all Home Health and DME orders to ensure patient receives services ordered.</li><li>
Provide extraordinary customer service to all internal and external customers (including patients and other</li><li>
ChenMed Medical team members) at all times. Utilization of patient messaging tools.</li><li>
Performs other related duties as assigned.</li></ul>
KNOWLEDGE, SKILLS AND ABILITIES:<ul><li>
Must be detail-oriented, possess the ability to multi-task and be open to cross-functionally training in referrals duties.</li><li>
The individual in this role must exercise proper phone etiquette and have the ability to navigate proficiently through computer software systems.</li><li>
Must be team-oriented and work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner.</li><li>
Exceptional organizational skills with the ability to effectively prioritize and timely complete tasks. Knowledge of medical terminology, CPT, HCPCS and ICD coding preferred.</li></ul><h2 class="jobSectionHeader">

EDUCATION AND EXPERIENCE CRITERIA:</h2><ul><li>
High School diploma or equivalent required</li><li>
A minimum of 1 year of referral experience in a healthcare setting required.</li><li>
Experience with web-based insurance sites and obtaining referrals/authorizations for multiple payors preferred.</li><li>
Experience with Web IVRs and obtaining referrals/authorizations for multiple payers strongly preferred</li><li>
Healthcare experience within the Medicare Advantage population preferred.</li><li>
Medical Assistant certification preferred</li><li>
CPR for Healthcare Providers is preferred</li></ul><div></div>
PAY RANGE:
$17.0 - $24.26 Hourly
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.
