</div><p>Care Coordination Department Description: Care Coordination consists of two different groups: Inpatient Care Coordination Nurses/ Discharge Planners and the Float Pool. The Care Coordination Nurses are assigned to either a unit or a particular population of patients. They work collaboratively with the providers and multi-disciplinary teams to provide education for patients and families, arrange transport to other facilities or home setting if needed, and set up post-discharge services (e.g., home health, DME, follow up appointments, outpatient testing, obtaining prescriptions or medications prior to discharge, and communicate with the primary care providers post discharge). The areas served by Care Coordinator Nurses include: Pediatric Hospital Medicine Group Complex Care Team/Clinic CV Services NICU Rehab/Ortho/ and Epilepsy unit Pulmonary Unit Diabetic Education for new onset patients Neurology Renal Behavioral Health Float Pool staff consist of RN&rsquo;s and Clinical Assistants that are used as supplemental staffing for the inpatient units during times of vacancies or high census. There are two different tracks in the float pool. Acute care Float Pool RN&rsquo;s may float to any acute or intermediate care area. Critical Care RN&rsquo;s float to any inpatient area as needed.</p>

Clinical Assistant Float Pool

<div><p>In Foundation and Development at Main Campus Lee Building</p><p>
Work schedule will be Full-time on the Day Shift, Mon-Fri Standard Days.</p><p>
Summary:</p><p><br>
The Development Operations Financial Coordinator works with the entire Foundation staff to provide support performing donor gift management and financial duties. The individual in this role must have high level technical and time management skills, as well as be an independent worker who takes ownership of the projects assigned. They must be able to perform a wide variety responsibilities and special projects while maintaining consistent attention to detail.</p><p></p><p>

Works under the supervision of licensed nursing staff to provide assistive care and communication for patients and their families in outpatient specialty clinics, as well as support for team members, professional and technical staff, and visitors. Provides safe, age-appropriate care and contributes to efficient clinic operations.</p><p></p><div>

The division of Pulmonary Medicine specializes in the treatment of patients with a variety of lung and sleep disorders. Specialty clinics include: General Pulmonary Medicine, Asthma, Cystic Fibrosis (CF), Technology Dependence/Home Ventilators, Bronchopulmonary Dysplasia (BPD), Primary Ciliary Dyskinesia (PCD), and Sleep (including Behavioral Sleep Disorders, Snore, and CPAP/BiPAP management). Board Certified Pulmonologists, Academic Fellows, Residents and Certified Registered Nurse Practitioners provide care in this department. Nurses, respiratory therapists, social workers, and nutritionists are also a part of the interdisciplinary team.</p></div><p></p>
