Position Summary
As a PRN Personal Care Partner / QMAP, you&rsquo;ll step in to support day and night coverage as needed. You&rsquo;ll meet daily personal care needs in a way that reflects our mission and values; anticipate, identify, and report care needs; and supervise and administer prescribed medications under the direction of a licensed nurse&mdash;following the &ldquo;five rights&rdquo;: right resident, right dose, right time, right medication, right route.
How You&rsquo;ll Help Residents Thrive<ul><li>
Champion daily living with dignity: Provide timely assistance with ADLs&mdash;bathing, dressing, grooming, oral hygiene, toileting, ambulation, laundry, and mealtime support&mdash;tailored to each resident&rsquo;s preferences (hair combed, shoes tied, etc.).</li><li>
See the person, not just the task: Anticipate and identify changing needs (e.g., hygiene, broken glasses, skin concerns, hearing aids, supplies) and report promptly.</li><li>
Be a steady, trusted presence: Maintain open, respectful communication with residents, families, and teammates; respond to family questions with warmth and professionalism.</li><li>
Promote safety and wellbeing: Monitor resident status and environment; document observations as directed; complete work orders for repairs or replacements.</li><li>
Uphold rights and protections: Immediately report suspected abuse, neglect, or misappropriation per regulation.</li><li>
Support medication citizenship:<ul><li>
Review MARs and administer medications accurately and on time, in accordance with state law and community policy.</li><li>
Document thoroughly, including refusals; restock medication carts/drawers with appropriate non-med supplies (gloves, cups, sanitizer); alert the RN/LPN or supervisor to any discrepancies, concerns, or supply issues.</li></ul></li><li>
Grow our shared culture: Participate in monthly staff meetings and contribute ideas that advance resident life and departmental excellence.</li><li>
Pitch in where needed: &ldquo;Other duties as assigned&rdquo; to support a thriving Community.</li></ul>
What You&rsquo;ll Bring<ul><li>

Experience in senior living, home care, or healthcare support (preferred)</li><li>
Current state-approved medication aide/assistant certification (as applicable)</li><li>
Familiarity with MARs and medication administration procedures; unwavering attention to detail</li><li>
Ability to document clearly, follow care plans, and communicate changes to nursing leadership</li><li>
Reliability, teamwork, and respect for confidentiality and resident rights</li><li>
Flexibility to accept PRN shifts across days, evenings, nights, weekends, and holidays</li></ul>

Qualifications 
Basic Qualifications &amp; Experience<ul><li>
Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation</li><li>
Must be able to read, write and speak the English language.</li><li>
High school or Equivalent (GED)</li><li>
One (1) year experience in some level of elder care preferred</li><li>
Proficient computer skills preferred</li><li>
Strong self-management and organization skills</li><li>
Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).</li><li>
Ability to effectively handle difficult and sensitive issues and maintain confidentiality</li></ul>
Working Conditions / Physical Requirements
<ul><li>Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.</li><li>
Constant standing, walking, and grasping. Frequent stooping and reaching above shoulder heights, occasional sitting, crawling, kneeling, and squatting.</li><li>
May have exposure to blood borne pathogens, body excretions and communicable diseases possible.</li><li>
Possible exposure to household chemicals.</li><li>
Sits, stands, bends, and moves intermittently during working hours.</li><li>
May be subject to frequent interruptions.</li><li>
Interacts with clients, family members and visitors frequently.</li></ul>
