This position offers an exciting opportunity to make a meaningful difference in the lives of those you serve while working in a dynamic, team-oriented setting.Responsibilities<ul><li>Assist residents with daily activities such as bathing, dressing, grooming, and mobility to promote independence and dignity.</li><li>Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, adhering to medical documentation standards.</li><li>Administer medications accurately following prescribed protocols while maintaining compliance with HIPAA regulations.</li><li>Support residents with specialized needs such as Alzheimer&rsquo;s care, dementia care, memory care, hospice care, and tube feeding procedures.</li><li>Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to document patient care activities thoroughly and accurately.</li><li>Perform basic nursing tasks including catheter care, wound dressing, and assisting with medical procedures under the supervision of nursing staff.</li><li>Collaborate with healthcare teams to ensure comprehensive patient care that aligns with physiology knowledge, anatomy understanding, and medical terminology.</li><li>Maintain a safe environment by adhering to infection control protocols and safety standards specific to assisted living facilities.</li></ul>Skills<ul><li>Proven experience working in assisted living facilities or nursing homes; hospital experience is highly valued.</li><li>Strong knowledge of senior care practices, including Alzheimer&rsquo;s, dementia, memory care, hospice, and home &amp; community care settings.</li><li>Familiarity with EMR/EHR systems such as Epic or Cerner for accurate medical documentation.</li><li>Excellent caregiving skills combined with a genuine passion for working with individuals with disabilities or chronic health conditions.</li><li>Ability to perform medical procedures such as tube feeding, medication administration, vital sign monitoring, catheter care, and basic anatomy/physiology knowledge.</li><li>Understanding of HIPAA regulations to ensure patient privacy and confidentiality at all times.</li><li>Effective communication skills to collaborate seamlessly with healthcare teams and family members while maintaining professionalism.</li><li>Capable of managing medical records accurately while demonstrating attention to detail in all aspects of patient care.
