We are an independent nonprofit organization dedicated to assisting Passaic County multiple-needs children and their families. As a Care Management Organization (CMO), we partner with families to design, implement, and manage individual service plans (ISPs) using both informal and community support for children with complex needs including behavioral, emotional, and developmental disabilities.

A DAY IN THE LIFE OF A CARE MANAGER SUPPORT SPECIALIST
As a Care Manager Support Specialist, you will be at the beginning of a rewarding career while helping children and families reach their goals. During the training period, you will also support Care Managers with driving and traveling by car approximately 50-70% of the time, as well as assisting with administrative tasks.
You will learn how to:<ul><li>
Use the Wraparound Model to form a Child-Family Team</li><li>
Coordinate both formal and informal supports</li><li>
Utilize family vision, crisis plans, strengths &amp; needs assessments, and care plans</li></ul>
You will be an integral part of fulfilling the goal of Circle of Care: Keeping youth at home, in the community and in school while creating self-sufficiency.
QUALIFICATIONS FOR A CARE MANAGER SUPPORT SPECIALIST<ul><li>
0-2 years of experience</li><li>
Bilingual Spanish, Arabic or Bengali preferred</li><li>
Bachelor's degree in a related field (Social Work, Psychology, Sociology, Criminal Justice)</li><li>
Valid driver's license with clean driving record</li><li>
Ability to work a flexible schedule including early morning and evenings as needed to meet with youth and families</li><li>
A heart for children with complex needs including behavioral, emotional, and developmental disabilities and their families</li></ul>
