 What You&rsquo;ll Do
 
 
 
 As an Adult Case Manager in our Community Support Services (CSS) Program, you&rsquo;ll be the bridge between individuals and the support systems they need to thrive. You&rsquo;ll help adults navigate mental health services, achieve goals outlined in their Individual Recovery Plans (IRPs), and foster connections to natural supports and community resources.
 
 
 
 Your Role Will Include
 
 
 
 <ul>
 <li>
 Conducting community needs assessments and providing personalized case management
 </li>
 <li>
 Collaborating with an interdisciplinary team to support treatment plans
 </li>
 <li>
 Facilitating communication between clients, families, and service providers
 </li>
 <li>
 Coordinating care, delivering psychosocial rehabilitation, and promoting community reintegration
 </li>
 <li>
 Documenting service contacts, maintaining ethical standards, and ensuring continuity of care
 
 
 </li>
 </ul>
 
 Your Responsibilities
 
 
 
 <ul>
 <li>
 Maintain consistent outreach (minimum twice/mnth per client)
 </li>
 <li>
 Deliver services in-person and via telehealth
 </li>
 <li>
 Attend community/agency meetings and represent client needs
 </li>
 <li>
 Communicate regularly with the Program Manager and attend required trainings
 </li>
 <li>
 Support clients&rsquo; recovery journeys with compassion, coordination, and clarity
 
 
 </li>
 </ul>
 
 What You Bring
 
 
 
 <ul>
 <li>
 Bachelor&rsquo;s degree in Human Services, Social Work, Counseling, Criminal Justice, or related field
 </li>
 <li>
 1+ years&rsquo; experience in behavioral health or case management (preferred)
 </li>
 <li>
 Tech-savviness and comfort using electronic records, phones, and telehealth tools
 </li>
 <li>
 Strong interpersonal and communication skills
 </li>
 <li>
 Reliable transportation and a valid driver&rsquo;s license (defensive driving training provided)
 </li>
 <li>
 A team-first mindset and a passion for helping others succeed
