Clarify is seeking a motivated and experienced <b>Physician Liaison</b> to engage directly with healthcare providers and practices to support Clarify&rsquo;s outcomes-based partnerships. In this role, you will guide providers/practices through sustained behavior change, including optimizing utilization of in-network, high-quality, and efficient care delivery. You will combine data-driven insights and structured change management approaches to build provider commitment and deliver measurable partnership outcomes.</div>

<p>As a Physician Liaison, you will serve as a trusted advisor to healthcare providers and practices, guiding them through a structured change journey to optimize utilization of in-network, high-quality care delivery. You&rsquo;ll leverage Clarify&rsquo;s platform and engagement playbooks to diagnose each provider or practice&rsquo;s readiness for change.</p>

<div><p>This role goes beyond traditional provider relations. You will function as a change agent, combining consultative relationship skills with a systematic, data-informed approach to influence providers/practices to achieve sustained behavior change.</p></div><div><br>

<div><p>Provider Partnership &amp; Relationship Management</p>
</div>
<div><ul><li><p>Build and maintain trusted, collaborative relationships with assigned providers (physicians, practice administrators, and staff)</p>

</ul></div><div><ul><li><p>Conduct regular interactions (both virtual and in-person) to review performance, co-develop improvement plans, and address barriers to change</p>

</ul></div><div><ul><li><p>Serve as the primary point of contact and advocate for assigned providers, representing their needs and perspectives internally</p>

</ul></div><div><p>Education &amp; Enablement</p>
</div>
<div><ul><li><p>Engage with practice administration, staff, and providers to support workflow improvement, education, and awareness on value-based care program requirements, quality measures, and performance incentives</p>
</li>
</ul></div><div><ul><li><p>Collaborate with internal teams to resolve barriers that may inhibit provider adoption</p>
</li>
</ul></div><div><p>Performance Analysis &amp; Insights</p>

<div><ul><li><p>Translate complex data into clear, actionable narratives tailored to each provider and/or practice</p>
</li>
</ul></div><div><ul><li><p>Monitor progress on initiatives, track leading indicators of behavior change, and adjust strategies based on results</p>

</ul></div><div><p>Documentation &amp; Engagement Tracking</p>

<div><ul><li><p>Maintain detailed records of interactions, change stage assessments, and engagement activities in CRM (Salesforce)</p>

<div><ul><li>Bachelor&rsquo;s degree in healthcare administration, Nursing, Public Health, Business, or related field</li>
</ul>
</div><div><ul><li><p>2&ndash;4 years of healthcare experience in one or more of the following areas:</p>

</ul></div><div><ul><li><div><ul><li><p>Provider engagement, network relations, or account management</p>
</li>
</ul></div></li></ul></div><div><ul><li><div><ul><li><p>Value-based care program operations or implementation</p>
</li>
</ul></div></li></ul></div><div><ul><li><div><ul><li><p>Healthcare consulting or practice transformation</p>
</li>
</ul></div></li></ul></div><div><ul><li><p>Proven ability to establish trust and credibility through partnership-oriented, non-transactional engagement</p>
</li>
</ul></div><div><ul><li><p>Proficiency in interpreting healthcare utilization data, cost trends, referral patterns, and quality metrics; comfortable working with analytics platforms and Excel</p>
</li>
</ul></div><div><ul><li><p>Excellent verbal and written communication with the ability to translate complex data into compelling, provider-specific narratives; comfortable presenting to both clinical and administrative audiences</p>
</li>
</ul></div><div><ul><li><p>Ability to independently manage a territory, prioritize activities, and drive results with minimal supervision</p>
</li>
</ul></div><div><ul><li><p>Proficient with CRM systems, analytics platforms, and structured engagement tools</p>
</li>
</ul></div><div><ul><li><p>Willingness to leverage AI-powered tools and insights to enhance engagement preparation, analysis, and strategy development</p>
