Overview We are seeking a dedicated and skilled T-LPN / LPN to join our healthcare team. This role involves providing high-quality nursing care across various clinical settings, including acute, long-term, and community-based care environments. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and medical documentation, with experience working with diverse patient populations such as seniors, individuals with developmental disabilities, and those requiring specialized care like Alzheimer&rsquo;s or dementia care. Proficiency in EMR/EHR systems such as Epic, Cerner, or eClinicalWorks is preferred. The T-LPN / LPN will play a vital role in delivering compassionate patient care while adhering to HIPAA and infection control standards.Responsibilities<ul><li>Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history documentation</li><li>Administer medications and injections accurately following physician orders and safety protocols</li><li>Perform IV infusions, catheterizations, and sterile procedures with aseptic technique</li><li>Manage airway support including ventilator care and airway management techniques</li><li>Assist with diagnostic evaluations such as laboratory specimen collection, processing, and sonography procedures</li><li>Provide specialized care for patients with conditions such as Alzheimer&rsquo;s disease, dementia, or behavioral health needs</li><li>Document all patient interactions thoroughly within EMR/EHR systems like Epic or Cerner</li><li>Collaborate with multidisciplinary teams for discharge planning, case management, and post-acute care coordination</li><li>Support patients in rehabilitation settings including ICU, PICU, NICU, or trauma centers</li><li>Educate patients and families on health coaching topics including nutrition, medication management, and chronic disease management</li><li>Ensure compliance with infection control policies and maintain sterile environments during procedures</li></ul>Experience<ul><li>Proven experience working in diverse healthcare settings such as hospitals (Level I Trauma Center preferred), emergency rooms, ICU/PICU/NICU units, outpatient clinics, or home &amp; community care environments</li><li>Strong background in critical care areas including ICU experience or emergency medicine is highly desirable</li><li>Familiarity with ICD-9/10 coding and CPT coding for accurate medical documentation</li><li>Experience administering injections, phlebotomy, venipuncture, catheter care, and tube feeding procedures</li><li>Knowledge of physiology and anatomy relevant to patient assessment and diagnostic evaluation</li><li>Hands-on experience with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is preferred</li><li>Ability to work with special populations including pediatrics (including neonatal ventilators), geriatrics (memory &amp; Alzheimer&rsquo;s care), behavioral health patients, or individuals with disabilities</li><li>Demonstrated understanding of HIPAA regulations and infection control standards to ensure patient privacy and safety</li></ul>This position offers an opportunity to work in a dynamic healthcare environment where compassionate patient care is prioritized. This role involves working closely with interdisciplinary teams to deliver comprehensive nursing services across various units, including acute care, geriatrics, and post-acute settings. The LPN will utilize their expertise in medical documentation, patient assessment, and clinical procedures to ensure safe and effective care delivery while adhering to HIPAA regulations and hospital standards.Responsibilities<ul><li>Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations.</li><li>Administer medications and injections accurately following physician orders and established protocols.</li><li>Assist with invasive procedures such as IV infusion, catheterization, venipuncture, and sterile dressing changes.</li><li>Monitor patients&rsquo; conditions using EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks; document all care activities thoroughly.</li><li>Manage airway care, ventilator support, and tube feeding for patients with complex needs such as those in ICU or PICU settings.</li><li>Conduct diagnostic evaluations including specimen collection/processing and basic medical imaging procedures.</li><li>Provide specialized care for patients with Alzheimer&rsquo;s disease, dementia, developmental disabilities, or behavioral health conditions.</li><li>Support discharge planning and coordinate post-acute care or home &amp; community care services.</li><li>Maintain infection control standards through aseptic techniques and proper sterilization procedures.</li><li>Educate patients and families on health coaching topics such as nutrition, medication management, pain control, and disease prevention.</li><li>Collaborate with multidisciplinary teams on case management for complex cases involving trauma centers or emergency medicine scenarios.</li><li>Ensure compliance with HIPAA regulations and maintain accurate medical records consistent with NCQA standards.</li></ul>Experience Candidates should possess prior experience in diverse healthcare settings such as hospital medicine, emergency room (ER), ICU, telemetry units, long-term care facilities, assisted living communities, or outpatient clinics. Experience with Epic EMR systems or similar EHR platforms is preferred. Familiarity with ICD-9/ICD-10 coding, CPT coding for procedures like suturing or dermal fillers, and medical terminology is essential. Additional desirable skills include proficiency in phlebotomy, sterile processing techniques, dialysis support, spinal tap procedures, gastrostomy management (Gastric lavage), and working with specialized populations such as pediatrics or neonatal ventilator patients. Candidates must demonstrate strong knowledge of physiology and anatomy principles relevant to nursing practice along with excellent patient assessment skills.
