What You'll Do</b></h3><ul><li>
Manage incoming referrals and coordinate admissions</li><li>
Review referrals for clinical appropriateness</li><li>
Communicate with patients, families, physicians, hospitals, and referral sources</li><li>
Verify insurance benefits and authorizations</li><li>
Coordinate seamless transitions of care</li><li>
Maintain accurate documentation and patient records</li><li>
Build and maintain strong relationships with referral partners</li></ul><h3 class="jobSectionHeader"><b>
Qualifications</b></h3><ul><li>
Current Washington State <b>LPN</b> license</li><li>
Healthcare experience required; home health, hospice, intake, referrals, or care coordination experience preferred</li><li>
Strong communication, organization, and customer service skills</li><li>
Ability to multitask and work independently in a fast-paced environment</li><li>
EMR and computer proficiency</li></ul><p><b>
