In collaboration with the Chief Health Officer (CHO), the Associate Chief Health Officer (ACHO) is responsible for the quality and performance of health services at CommUnityCare. As such, the ACHO directly oversees the service lines to which they are assigned, helping establish the organization's clinical standards of practice and integrated, high-performance model of care. Additionally, through expert collaboration, the ACHO teams with quality and performance improvement, clinical informatics, operations, finance, compliance, and nursing leadership to secure a highly-reliable, innovative, teambased care environment that advances health equity, continuously improves quality, builds community, and promotes the wellbeing of our workforce &ndash; all with demonstrable ingenuity and resourcefulness. Within this scope, the ACHO oversees and leads clinical quality improvement, disease management, utilization review, clinical outcomes, preventive health, research and education, clinical integration, and new market development. Imbued with enthusiastic support of value‐based, effective, efficient, and team‐based care across the entire care delivery system continuum, ACHO sets a transformative standard of excellence in clinical leadership. Responsibilities:

Works with the CHO to implement a strategic direction of the organization related to health services. Works with the CHO and other leadership to establish and/or maintain an environment of health and wellness for everyone: patients, staff and community. Works with clinical leadership, departmental leads, and community thought leaders to prioritize, integrate and support existing clinical programs as well as new initiatives. Fosters a culture of best demonstrated practices, measurement, performance, continuous improvement, accountability, customer service, as well as exemplary teamwork. Assists in the development, implementation and maintenance of all treatment and prevention services as well as population health programs in accordance with federal, state, and local community health regulations and standards. Oversees clinical programs as assigned, including disease management, high risk programs, care management, population health, care model advancement, wellness and prevention and other programs as designed to achieve CommUnityCare&rsquo;s mission and vision. Associate Chief Health Officer (Primary Care)

<div>Overview:

The Pharmacist Lead &ndash; Specialty &amp; Infusion Operations is responsible for the operational, clinical, regulatory, and<br>
financial oversight of specialty pharmacy and ambulatory infusion pharmacy services. This role includes oversight<br>
of specialty and infusion services delivered within a licensed outpatient retail pharmacy, supporting prescription<br>
dispensing within the pharmacy, including specialty medications and sterile and non-sterile compounded therapies,<br>
and contributing to both patient care and organizational revenue generation. This position serves as the designated<br>
pharmacy lead for specialty medications, infusion therapy programs, specialty and infusion-specific patient access<br>
support functions, and related 340B optimization opportunities. The role operates in close collaboration with the<br>
Director of Outpatient Pharmacy for program strategy and performance, and with the Pharmacist-in-Charge to<br>
ensure compliance with pharmacy operations, regulatory requirements, and dispensing standards. The Pharmacist<br>
Lead collaborates with providers, nursing, pharmacy operations, revenue cycle, and informatics teams to ensure<br>
safe, compliant, and patient-centered medication management while supporting organizational growth and<br>
sustainability. This position provides direct patient care, oversees specialty and infusion pharmacy workflows, leads<br>
quality improvement initiatives, and serves as a subject matter expert for specialty medications, infusion therapies,<br>
payer requirements, and medication access programs.</div>

Oversees daily operations of specialty and infusion pharmacy services within the<br>
licensed outpatient pharmacy, including prescription processing, verification, dispensing, and patient counseling.<br>
Ensures safe and efficient workflow for specialty and infusion-related medications, including sterile and non-sterile<br>
compounded products in accordance with USP &lt;797&gt; and &lt;800&gt; standards.<br>
Maintains regulatory and operational compliance with all dispensing, labeling, and documentation requirements.<br>
Optimizes staffing models, workflow design, production capacity, and inventory mangement to support prescription<br>
volume, patient access, and financial performance.<br>
Partners with the Pharmacist-in-Charge to ensure alignment with pharmacy-wide policies, regulatory<br>
requirements, and operational standards. Functions within the licensed pharmacy under the supervision and<br>
regulatory authority of the Pharmacist-in-Charge.<br>

procurement, prescription fulfillment, dispensing, patient onboarding, refill management, and adherence<br>

Develops and maintains specialty medication workflows and service standards; monitors adherence, turnaround<br>
times, patient retention, and financial performance; and coordinates with manufacturers, specialty distributors,<br>
and payer organizations to support medication procurement and reimbursement processes.<br>
Oversees specialty medication procurement strategy, including sourcing through wholesalers, specialty<br>
distributors, and 340B channels; manages inventory levels, product selection, and purchasing decisions to optimize<br>
cost, reimbursement, and product availability; and partners with supply chain and finance teams to support<br>
contract optimization and margin performance.</div><div><b><br>
Infusion Pharmacy Program Management:
</b></div><div>Leads and is accountable for pharmacy-related aspects of ambulatory<br>
infusion services, including oversight of sterile compounding activities and preparation of infusion medications in<br>
accordance with USP &lt;797&gt; and &lt;800&gt; standards, storage, inventory management, and administration support.<br>
Ensures compliance with USP &lt;797&gt;, USP &lt;800&gt;, state and federal regulations, and accreditation standards;<br>
collaborates with nursing leadership on infusion workflows, scheduling, and monitoring processes; and supports<br>
implementation of new infusion therapies and service line expansion.</div><div><b><br>
Clinical Leadership:
</b></div><div>Provides clinical oversight and advanced practice leadership for specialty and infusion<br>
medication services; reviews and verifies medication orders for appropriateness, safety, and compliance; develops<br>
treatment plans, monitoring parameters, and clinical workflows; collaborates with providers and nursing staff to<br>
optimize therapy outcomes; serves as a resource for complex medication questions; and monitors adverse drug<br>
reactions, infusion reactions, and medication safety events.</div><div><b><br>
Medication Access and Financial Stewardship:
</b></div><div>Partners with medication access, prior authorization, and medication<br>
assistance teams, as well as revenue cycle functions, to support specialty and infusion-specific patient access<br>
support functions and improve therapy initiation and continuity of care.<br>
Identifies opportunities to maximize 340B program benefits while maintaining compliance; monitors drug<br>
utilization trends; optimizes inventory management strategies; assists with specialty and infusion contracting<br>
initiatives; and participates in budget development and financial performance reviews.<br>
Quality and Regulatory Compliance: Ensures compliance with Texas State Board of Pharmacy regulations, USP<br>
