Job Summary
 
The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.
 
Skills/Requirements
 <ul>
<li>Highschool diploma or GED required. Associates degree of higher in a field related to behavioral health preferred.</li>
<li>1-3 years of experience working with the GMH/SU population required.</li>
<li>1-3 years of experience in behavioral health position providing outreach and engagement activities required.</li>
<li>1-3 years of experience in a medical setting preferred.</li>
<li>Minimum of 6 months of recovery from substance use and/or mental health disorders preferred.</li>
<li>Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.</li>
<li>Valid Identification required.</li>
<li>Current AZ Driver’s License (valid and in good standing) preferred.</li>
<li>Clean 39-month driving record preferred</li>
<li>Arizona Fingerprint Clearance Card.</li>

Job Summary
 
The Specialty Case Manager provides case management services in the community to individuals who have a mental health and/or substance use diagnosis, survivors of domestic violence, and/or those who are considered high risk due to their acuity level. The Specialty Case Manager assists in planning, connecting, advocacy, coordination and monitoring; to assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports. They asses needs, develop and monitor individualized treatment plans and document services provided in an accurate and timely manner. The Specialty Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The Specialty Case Manager will be responsible for implementing advanced techniques and interventions to serve the high-risk individuals in their associated program.
 
Skills/Requirements
 
<ul>
<li>Highschool diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.</li>
<li>Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred</li>
<li>1-3 years of experience in a behavioral health position providing outreach and engagement activities preferred.</li>
<li>1-3 years of experience working with special populations (individuals experiencing serious mental illness, survivors of domestic violence, individuals determined high cost/high need preferred.</li>
<li>Or equivalent combination of education and experience</li>
<li>Current AZ Driver's License (valid and in good standing).</li>
<li>39-month MVR</li>
<li>Peer Certification (if applicable)</li>
<li>Arizona Fingerprint Clearance card</li>
<li>Jail Clearance (if applicable)</li>
