Position Summary
 The Advanced Practice Registered Nurse collaborates with physicians and interdisciplinary teams to provide comprehensive palliative and hospice care in various settings. The APRN delivers expert clinical evaluation, treatment, and management of patients with serious or terminal illnesses. Responsibilities include assessing patient needs, facilitating goals of care discussions, prescribing treatments, and supporting transitions of care. The APRN plays an integral role in educating staff, families, and the broader medical community about palliative and hospice care.
 Position: 32 hours a week/4-day work week. Must be able to work 1 or 2 weekend days a month.
 Position located in North Central Florida, Gainesville area.
 
Essential Functions
 <ul>
<li>Provide medical expertise in pain and symptom management, including terminal prognosis evaluation.</li>
<li>Act as attending provider in hospice contract beds in collaboration with the hospice physician.</li>
<li>Coordinate admissions and discharges for hospice contract beds, including review of terminal diagnoses.</li>
<li>Assess patient conditions, develop care plans, implement interventions, and evaluate outcomes.</li>
<li>Conduct patient and family assessments, ensuring readiness for transition to hospice or home settings.</li>
<li>Perform physical exams, diagnostic testing, treatment, follow-up, and consultations.</li>
<li>Prescribe and administer medications and other therapeutic interventions.</li>
<li>Collaborate and consult with hospice/palliative physicians, attending physicians, and interdisciplinary team.</li>
<li>Support patients and families in clarifying care goals and end-of-life decisions.</li>
<li>Maintain accurate and complete medical records to ensure compliance and reimbursement.</li>
<li>Ensure informed consent documentation, including CPR and DNR discussions.</li>
<li>Participate in interdisciplinary team meetings and on-call rotations.</li>
<li>Educate internal staff and community members on palliative and hospice care.</li>
<li>Coordinate inpatient APRN assignments and support departmental functions.</li>
</ul>
Qualifications
 
Knowledge and Critical Skills
 <ul>
<li>Expertise in patient charting, history taking, and documentation practices.</li>
<li>Knowledge of health education methods, clinical operations, and medical procedures.</li>
<li>Proficient in diagnosis, treatment planning, and symptom management.</li>
<li>Excellent interpersonal, verbal, and written communication skills.</li>
<li>Ability to organize, analyze, and prioritize complex tasks and data.</li>
<li>Comfortable with public speaking, leadership, and team collaboration.</li>
</ul>
Experience
 <ul>
<li>Minimum two (2) years of nursing experience.</li>
<li>Experience in hospice or palliative care preferred.</li>
</ul>
License / Certification
 <ul>
<li>Current Advanced Practice Nurse or Physician Assistant license in the State of Florida.</li>
<li>Valid Florida driver’s license with acceptable driving record and reliable transportation.</li>
</ul>
Education
 <ul>
<li>Master’s degree from an accredited Advanced Practice Nursing program.</li>
</ul>
Other Requirements
 <ul>
<li>Medical clearance for communicable diseases prior to patient contact.</li>
<li>Up-to-date immunizations including, but not limited to, TB, Hepatitis, and Influenza.</li>

The Housekeeper position maintains assigned areas in a clean, neat and sanitary manner. S/he performs facilities maintenance duties including dusting, mopping, laundry, finishing and buffing floors, vacuuming and shampooing carpets, cleaning and restocking restrooms.
 Position Details
 <ul>
<li>Position Location: Community Hospice and Palliative Care, 4266 Sunbeam Road</li>
<li>Days/Hours: Part-Time 20 hours a week; Monday - Friday, Nights only 5:00pm - 10:00pm</li>
<li>Compensation: $16.00 hour</li>
<li>Requirement: *MUST* have reliable, daily vehicle transportation (i.e., personal car) due to travel between company sites; must be able to successfully pass a level two background check</li>
</ul>
Job Responsibilities: 
<ul>
<li>Thoroughly cleans all assigned patient rooms, departments, and Community Hospice facilities in accordance with Community Hospice’s established cleaning and sanitary procedures.</li>
<li>Responsible for maintaining an adequate supply of clean linens for patients by laundering and sanitizing of soiled linens, when scheduled for laundry rotation.</li>
<li>Ensures proper care in the use and maintenance of equipment and supplies.</li>
<li>Promotes continuous improvement of workplace safety and environmental practices.</li>
<li>Provides emergency cleaning of corridor carpets, upon request.</li>
<li>Provides assistance during emergency situations as outlined in the facilities fire and disaster plans.</li>
<li>Adheres to all infection control and isolation procedures to ensure a safe work environment.</li>
<li>Understands and demonstrates proper usage of all cleaning chemicals in accordance with the MSDS product standards and Community Hospice procedures.</li>
<li>Completes all scheduled daily assignments within designated time frames. Informs manager at the end of the day of any assignments not completed.</li>
<li>Projects quality customer service to internal and external customers at all times.</li>
</ul>
Experience & Knowledge: 
<ul>
<li>Previous housekeeping experience is preferred.</li>
<li>Cleaning and safety equipment is helpful.</li>
<li>Standard Housekeeping procedures.</li>
<li>Ability to operate vacuums, floor extractors, and other power tools applicable to the position.</li>
</ul>
