<div><div><div><div><b>Constellation Health Services is looking for a compassionate and skilled </b><b>Per Diem Registered Nurse</b><b> to work with hospice patients and their families in their homes, assisted living facilities, and in skilled nursing facilities.
</b></div></div><div><div><b>Locations to be covered include Butler, Warren, Hamilton, Brown and Claremont Counties.</b></div></div><div><div><b>
Hours would be M-F, daytime shifts 8am - 5pm.</b></div></div></div><p>

</p><p><b>Responsibilities:
</b></p><ul><li>Provides skilled nursing care including the administration of medications and treatments as prescribed in the physician's plan of care for patients in their home setting.</li><li>
Performs a comprehensive assessment and coordinates the plan of care with all disciplines associated with the patient's needs, including family and primary caregivers.</li><li>
Observes, evaluates and reports on the patient's physical and emotional status to the nursing supervisor and the attending physician whenever a change occurs.</li><li>
Participates in weekly Interdisciplinary Team Meetings, providing updates on patient's physical, emotional, psychosocial, and spiritual status.</li><li>
Maintains and processes all documentation in compliance with agency standards</li><li>
Will utilize a computer-based documentation system and submit all required clinical documentation, time records and other required information in the required time frame.</li><li>
Provides input to Home Health Aide Supervisor as needed for purpose of hospice aide evaluation.</li><li>
Weekend &amp; holidays coverage (as needed)</li></ul><p>

Qualifications:</b></p><ul><li>
Graduate of an accredited RN program.</li><li>
Registered Nurse Licensed in the state of employment.
</li><li>At least 1-2 years of RN experience is a<b> MUST</b></li><li>
Previous Hospice care strongly desired</li><li>
Basic knowledge of iOs operating system</li></ul><p>
