Who We Need</b></div><div>
The ACT Team Psychiatric Nurse Practitioner is responsible for the provision of evidence-based and coordinated mental health services to participants in this program. The ideal candidate should have the ability to coordinate and work with a team of case management specialists in the screening of new referrals to the service, engagement of participants, comprehensive assessment, diagnosis, treatment planning, medication management, counseling, patient education, and risk assessment. The job requires coordination with the provider of medical care for the participants including their outpatient medical follow up as well as inpatient hospitalizations.</div><div></div><div><div><b>

JOB SUMMARY (INCLUDING, BUT NOT LIMITED TO)</b></div><div><ul><li>
Serves as the Psychiatric Nurse Practitioner of all participants on the ACT Team and provides completes initial psychiatric assessments, progress notes, annual psychiatric updates, family interventions, develops differential diagnostic assessments, assessments of capacity for consent to treatment, risk assessments, patient education, progress towards treatment goals including medical, psychological, social, behavioral and developmental aspects of the service participants&rsquo; situation.<br>
</li><li>
Promotes and advances the principles and practices of recovery oriented treatment of ACT participants.<br>
</li><li>
Documents participant's progress in the Electronic Health Record.</li><li>
Maintains accurate, thorough, and current documentation in accordance with policies, procedures, and clinical standards of Copa Health. Consult complex cases with peers within the network and provide consultation and second opinions as needed.<br>
</li><li>
Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for prior authorization for non-formulary medications as needed.<br>
</li><li>
Discusses and follows appropriate process of obtaining consents for use of psychopharmacological interventions and other modalities of treatment.<br>
</li><li>
Orders follow-up blood levels and supportive laboratory tests according to standard medical practice.<br>
</li><li>
Participates in daily communication occurring at a level necessary to provide care for all service participants served at a level required for continuity of care across team principles.<br>
</li><li>
Assesses service participants&rsquo; needs and guides the clinical team while managing treatment plan updates, in addition to initiating contact with outside providers according to established guidelines.<br>
</li><li>
Available on-call 24 hours/day, 7 days/week unless otherwise scheduled as part of a rotation or in event of a scheduled absence in which coverage has been arranged.<br>
</li><li>
Adhere to evidence-based practice and fidelity standards as required by AHCCCS.<br>
</li><li>
Actively participates in clinical performance initiates to improve the quality of care of providers.<br>
</li><li>
Provides necessary information to clinical team regarding inpatient utilization. Utilizes appropriate medication based on best practices, in addition to appropriately completing required documentation for preauthorization of pharmacy services.<br>
</li><li>
Participates in all Quality Improvement activities as required including but not limited to polypharmacy.<br>
</li><li>
Works closely with other personnel to ensure that each treatment modality is appropriate for the service participants in addition to working with outside psychiatric providers in crisis and inpatient services.<br>
</li><li>
Provides guidance in the process of obtaining, maintaining, or terminating court mandated treatments for participants.<br>
</li><li>
Participates in medical staff meetings monthly medical staff meetings.<br>
</li><li>
Works with Copa Health&rsquo;s Medical Management and Quality Management to complete all deliverables (ACT Fidelity reports, COT reports, etc.). Participates in monthly and ad hoc meetings to address participants on the Copa Health High Risk and Emerging Risk Registries.</li><li>
Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols.<br>

Performs quality checks on all eligibility initiated and ensures there are process for all members who do not a valid, accurate, current insurance.</li><li>

Investigates and resolves denial cases of members who lose eligibility.</li><li>

MINIMUM REQUIREMENTS:</b></div><div>

Who We Need</b></div><div>
As a member of the Assertive Community Treatment (ACT) Team, the ACT Housing Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. Works as a team member of a clinical team to assess consumer needs and wants, participates in the development of individual service plans, acts as a consumer advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Will serve as the ACT&rsquo;s specialist on housing and collaborates with other ACT members to provide housing and housing related services for ACT consumers. Provides on-call crisis intervention services, as well as weekend and holiday coverage on an established schedule. Flexible work schedule is required.</div><div><b>
MINIMUM QUALIFICATIONS:</b></div><div>
Requires anyone (1) combination of education and years of experience below:</div><ul><li>
Master&rsquo;s degree in a behavioral health related field.</li><li>
Bachelor&rsquo;s degree in behavioral health related field required. Two (2) years behavioral health work experience preferred.</li><li>
Associate degree in behavioral health related field and four (4) years behavioral health work experience.</li><li>
Has high school diploma or equivalent and eighteen (18) credit hours of post-high school education in a field related to behavioral health completed no more than four (4) years before the date of proposed employment and two (2) years full time behavioral health experience OR four (4) years of behavioral health work experience.</li><li>
Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.</li><li>
Ability to meet Copa&rsquo;s driver authorized driving requirements, set forth by Copa Health&rsquo;s insurance company. Including AZ Driver&rsquo;s license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable.</li></ul><div><b>
JOB SUMMARY (Including, but not limited to)</b></div><ul><li>
 Works as part of a clinical team to facilitate recovery and inspire hope for consumers while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan, regardless of the consumer&rsquo;s current location.</li><li>
 Meets with the consumers in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.</li><li>
 Works with consumer&rsquo;s Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individual&rsquo;s service plan (including all necessary assessments) and their crisis plan.</li><li>
 Coordinates with all parties involved in consumer&rsquo;s treatment according to policy and procedure and clinical protocols, including but not limited to, attending inter and intra-agency staffing, communication by telephone and letter, service/support delivery etc., as indicated on the consumer&rsquo;s service plan.</li><li>
 Responsible for transporting consumers to necessary services, as outlined in their service plan, as needed.</li><li>
 Assumes &ldquo;primary contact&rdquo; responsibility for a percentage of team consumers to: assure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individuals; keep weekly client schedule for those individuals updates, and act as first contact for those consumers and their families.</li><li>
