Who We Need</b></p><p><b>
The Lead Registered Nurse Serves as an active member of the site management team working collaboratively with the Site Administrator, Clinical Director and the Chief Psychiatrist (and other leadership) in regards to the site-based delivery of clinical care. Assists the Site Administrator, Clinical Director, Chief Psychiatrist and CMO/VPCS to ensure effective coordination and communication with the site nursing staff. Assists the Site Administrator with operational issues at the site that affect clinical nursing practices. Assists in communicating system initiatives to nurses and medical technicians. Provides clinical supervision to all site nursing staff which includes: RNs, LPNs and medical technicians including contract staff. Reports to the CMO/VPCS primarily for clinical issues and the Site Administrator for primarily administrative issues.</b></p><p></p><p><b>
JOB SUMMARY (Including, but not limited to)</b></p><div><ul><li><b>
The Lead Nurse carries a reduced caseload consistent with the number of clinicians supervised.</b><br>
</li><li><b>
Responsible for performing the essential duties and responsibilities for a site nurse.</b><br>
</li><li><b>
Serves as a member of the site management team working collaboratively with the Site Administrator, Clinical Director and the Chief Psychiatrist (and other leadership). Helps the Site Administrator, Chief Psychiatrist and Clinical Director to ensure effective coordination and communication with the site nursing staff. Serves as the point person for the site nursing staff and the Site Administrator regarding clinical operational matters specific to scope of work of the nursing staff.</b><br>
</li><li><b>
Records potential barriers to quality nursing care identified by the site nurses and collaborate with the Site Administrator, Clinical Director, Chief Psychiatrist and the CMO/VPCS in finding solutions to overcoming these barriers.</b><br>
</li><li><b>
Clinically supervises all RNs, LPNs, medical technicians including contract RNs, LPNs and medical technicians working at their site. This includes participating in performance reviews (Lead Nurse will address clinical performance and Site Administrator will address administrative performance), being available for clinical consultation and support, participating in peer reviews and corrective action plans as required.</b><br>
</li><li><b>
Provides direct oversight for all regulatory requirements associated with the campus laboratories, medication storage and disposal, and related medication services. This includes but is not limited to routine inspections, audits around documentation of assistance in the self-administration of medication, and overall compliance with regulatory standards in maintaining laboratories and medication storage areas.</b><br>
</li><li><b>
Supports and participates in peer reviews, grand rounds, monitoring activities, and corrective action plans, performance improvement projects intended to address the delivery of quality nursing care and identified standards of practice.</b><br>
</li><li><b>
In coordination with the CMO/VPCS and Site Administrator, participates in the orienting and mentoring of new nurse staff members to general nursing and site-based job performance expectations and standards of practice.</b><br>
</li><li><b>
Participates in the recruiting and retention efforts pertaining to site nursing staff as requested.</b><br>
</li><li><b>
Responsible for promoting and role modeling practice according to the recovery model and Arnold v. Sarn principles; knows and practices engagement skills including motivational interviewing; and ensures all nursing staff assist consumers to define and achieve their own recovery goals.</b><br>
</li><li><b>
Participates in decentralized UM and QM activities as appropriate and necessary.</b></li></ul><p><b>
Minimum Requirements</b></p><ul><li>
RN Diploma, AA or BSN preferred.</li><li>
 3-5 years of work experience. Minimum of 5 years behavioral health nursing experience with a minimum of one year experience in an outpatient setting.</li><li>
 Required: Car, valid AZ Driver&rsquo;s License, state minimum insurance coverage and ability to meet and maintain the company&rsquo;s acceptable driving requirements.</li></ul><p></p><p><i>

Who We Need</b></div><div>
The ACT employment specialist provides direct treatment, rehabilitative and supportive services in a community setting to ACT members. Provides direct job development services and employment support services to assist to obtain and maintain community-based employment. Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage.</div><div></div><div><div><b>
MINIMUM QUALIFICATIONS:</b></div></div><div><ul><li>
High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health or<br>

Bachelor&rsquo;s Degree in a related field OR a bachelor&rsquo;s degree in a non-related field with one (1) year experience preferred.<br>

Must have 1 year of training or experience in vocational rehabilitation and support. Prior experience may be job development, job coaching, or supported employment.<br>

Must have a Level One Fingerprint Clearance Card by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire.<br>

TB Test clearance required at the time of hire.<br>

Conduct vocational assessments to identify members&rsquo; skills, interests, and employment goals.<br>

Develop individualized employment plans in collaboration with the members, focusing on skill development, job readiness, and job placement.<br>

Provide vocational counseling, coaching, and support to members in job search strategies, resume writing, interview preparation, and workplace behavior.<br>

Establish partnerships with employers, vocational training programs, and community organizations to create job opportunities for clients.<br>
</li><li>
Collaborate closely with other members of the ACT team, including Rehabilitation Specialists, BHMP, and clinical team to coordinate comprehensive care and support for the members.<br>

Meets with the members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed.<br>
</li><li>
Works with members of the clinical team to ensure the timely development and implementation of the individual&rsquo;s service plan (including all necessary assessments) and their crisis plan.<br>
</li><li>
Coordinates with all parties involved in member&rsquo;s treatment according to policy and procedure and clinical protocols, including but not limited to, attending inter and intra-agency staffing, communication by telephone and letter, service/support delivery etc, as indicated on the consumer&rsquo;s service plan.<br>
</li><li>
Assists members and families in accessing additional needed services and obtaining entitlements, including identification of funding services.<br>
</li><li>
Responsible for transporting members to necessary services, as outlined in their service plan, as needed.<br>
</li><li>
Assumes &ldquo;primary contact&rdquo; responsibility for a percentage of ACT Team members to: ensure maintenance of accurate, thorough, and current documentation in the medical record (in accordance with policies, procedures, and clinical protocols) for those specific individual<br>
</li><li>
Keep weekly member schedule for those individuals updates, and act as first contact for those members and their families.<br>
</li><li>
Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols, and meets/exceeds member engagement goals as defined by Copa Health.<br>

Participates in daily team meetings as well as any additional individual treatment teams or other team communication meetings as required.</li><li>
Works in collaboration with team members to assure compliance with ACT fidelity measures.<br>

The ACT Peer Support Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. Works as a team member of a clinical team to assess consumer needs and wants, participates in the development of individual service plans, acts as a consumer advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the team&rsquo;s lead consumer advocate and mentor. Is a self-disclosed mental health consumer and is willing to provide a consumer&rsquo;s perspective of the team&rsquo;s service provision.</p><p><b>
MINIMUM QUALIFICATIONS:</b></p><ul><li>
High School Diploma, or GED 1-4 years of behavioral health experience</li><li>
Must be 21 years or older</li><li>
Previous experience working as a Peer Support Specialist is highly preferred.</li><li>
Peer Certification required upon hire or within 30 days of starting.</li><li>
AZ approved Peer Employment training program and have recent certification as of 2013.</li><li>
Must have basic knowledge and understanding of mental illness.</li><li>
Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.</li><li>
Ability to meet Copa&rsquo;s driver authorized driving requirements, including AZ Driver&rsquo;s license upon hire, 39-month MVR.</li></ul><p><b>
