Location: Field based - Southwestern, Ohio (Hamilton, Clinton, Clermont, Butler, and Warren Counties) 
Job Type: Full-Time (37.5 hours/week) 
Pay Range: $49,000 &ndash; $66,000 per year (commensurate with experience)

At COA, RN Care Managers help older adults and caregivers navigate life&rsquo;s challenges with dignity, compassion, and clinical expertise. You&rsquo;ll build long-term relationships, coordinate care, and empower clients to live independently&mdash;all while maintaining control over your schedule and caseload. 


What You&rsquo;ll Do
<ul><li>
Conduct in-home assessments and develop individualized care plans</li><li>
Coordinate services and monitor client progress</li><li>
Advocate for older adults and their caregivers</li><li>
Collaborate with interdisciplinary teams and community partners</li><li>
Maintain accurate documentation and ensure compliance with program standards</li></ul> 


What You&rsquo;ll Bring
<ul><li>
Active RN license in the State of Ohio</li><li>
Minimum 1 year of clinical experience in home health, community health or geriatrics</li><li>
Strong communication and organizational skills</li><li>
Ability to work independently and manage a flexible caseload</li><li>
Valid driver&rsquo;s license and reliable transportation</li></ul>
