<div><h2 class="jobSectionHeader"><b>Typical Tasks</b></h2></div>
<p></p><div><ul><li>Takes routine temperature, pulse and respiration of patients;<br>
</li><li>
Assists and encourages patients with the basic activities of daily living (ADLs), such as bathing, dressing, eating, toileting, positioning, lifting or other essential needs;<br>
</li><li>
May provide total care, as necessary under direction of a RN;<br>
</li><li>
As a member of a multidisciplinary team, and in compliance with approved techniques, intervenes in crisis situations by identification, prevention, early intervention, and if indicated, containment of assaultive behaviors in accordance with County training provided to Mental Health Workers;<br>
</li><li>
Reports changes in physical condition and/or behavior of patients to the RN;<br>
</li><li>
Monitors and records patient activities, maintains special observations and precautions;<br>
</li><li>
Engages patients in individual and/or group activities and conversation;<br>
</li><li>
Assists in orienting new patients to the unit;<br>
</li><li>
Monitors the milieu for safety; reports any unsafe conditions to a RN;<br>
</li><li>
Searches patient belongings for contraband upon admission, and/or as directed;<br>
</li><li>
Accompanies patients to on-site appointments;<br>
</li><li>
Maintains patient confidentiality;<br>
</li><li>
May arrange transportation for discharged patients;<br>
</li><li>
May maintain visitors log and coordinate patient visits;<br>
</li><li>
May order and restock supplies and maintain supply sign-out log;<br>

May update, maintain and provide community resources to patients, visitors and family members;<br>

Obtains and delivers specimens to laboratory for analysis;<br>

May be assigned as a Disaster Service Worker, as required;<br>

<p></p><div><div>Possession of a high school diploma or equivalent and one (1) year of experience providing patient care in an in-patient acute or sub-acute psychiatric facility.</div><div>

College course work in Sociology and Psychology is desirable.</div><div>

Possession of a CPR for the Professional Rescuer Card before completion of Acute Psychiatric Services orientation.<br>

Mental Health Worker

<div>JOB Under general direction, provides skilled social work services to cases requiring maximum service, and involving a variety of complex social problems and treatment plans.<br>
<br>
<b>Social Worker III positions in the Social Services Agency, Department of Family and Children's Services work in a variety of assignments connected to child safety, well-being, and permanency. The Social Worker III duties may include: receiving and making reports of alleged child abuse and/or neglect; assessing and investigating allegations of abuse and/or neglect; developing appropriate interventions, providing referrals for services, and monitoring services for children and parents; removing children from unsafe environments; providing case management services; preparing documentation, case plans, safety plans, Court Reports, and other legal documents; providing testimony in Court; assessing and evaluating potential caregivers; placing children into appropriate homes; conducting home visits with children, caregivers, and parents; and assessing and addressing issues specific to the children and families circumstances such as substance abuse, intimate partner violence, mental health issues, sexual abuse, parenting, developmental issues, immigration, and psycho-social-emotional-behavioral issues.</b><br>

Social Worker III - Department of Family and Children Services (DFCS)

<div><p>Under direction, to assess, plan, implement and evaluate education and to provide other health services for a specific patient population; to function as a consultant and educator to staff.<br>
<br>
<b>Custody Health Services (CHS) provides medical, mental health, and dental services to patients in custody at the 4 County Jail facilities: Main Jail, Elmwood, Juvenile Hall, and James Ranch. CHS provides on-site routine and urgent/emergent medical services at both Main Jail and Elmwood adult detention facilities in Santa Clara County. Health care of incarcerated men and women includes care of acute and chronic illness as well as preventative and specialty care. CHS also runs a 40-bed acute psychiatric inpatient housing unit at the Main Jail. The services are provided for adult male and female patients who require acute inpatient services for stabilization. Juvenile Custody Health provides integrated medical services to justice involved youth housed in Juvenile Hall and in James Ranch to include comprehensive pediatric medical, mental health, and dental care.</b><br>

<div><p>Possession of the legal requirements to practice as a Registered Nurse as determined by the California Board of Registered Nursing and sufficient education and experience for a candidate to be able to demonstrate possession of the qualifications necessary to successfully perform the typical tasks noted above.<br>
<br>
<b>Experience Note:</b> A candidate would typically acquire the education and experience necessary to successfully perform at this level by having graduated from a four year college or university with a BSN. Possession of a Master's Degree (MSN) is desirable. A qualified applicant would typically possess three years of clinical experience in the specialty area.</p></div></div></div>
