Title: Service Coordinator (Mental Health Case Manager)

Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Qualifications:
<ul>
<li>Two (2) years of full-time experience or part-time equivalent in a mental health setting.</li>
<li>A bachelor's degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.</li>
<li>Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.</li>
<li>Familiarity with regulatory standards.</li>

Title: Clinic Nurse (Shift Supervisor) Job Duties: The Clinic Nurse (Shift Supervisor) assesses, plans, organizes, develops, implements, and evaluates all the activities related to nursing standards of care, professional development, operational function, and communication throughout the facility. These activities will be performed in compliance with the mission, philosophy, and policies of Crestwood Behavioral Health Center. The Clinic Nurse (Shift Supervisor) actively participates in and adheres to all organizational and departmental goals and objectives. The Clinic Nurse (Shift Supervisor) is responsible for the client care delivered on the clinical unit. The Clinic Nurse (Shift Supervisor) reports to the Director of Nursing. Licensed Vocational Nurse (LVN) | Licensed Psychiatric Technician (LPT) encouraged to apply. Schedule: Full-Time Qualifications: Current licensure as an LVN or LPT in the State of California. Bachelor's degree preferred. Current CPR and first aid certification.
