We are seeking a Licensed Psychologist to join our integrated clinical team. In this role, you will work onsite in one or more long-term care facilities within your local community, providing therapy, assessment, care planning, and behavioral health support. This position offers professional autonomy, a predictable Monday&ndash;Friday schedule, and robust administrative and clinical support so you can focus on what matters most&mdash;helping patients thrive.

What You&rsquo;ll Do

As a key member of the interdisciplinary team, you will:

Clinical Services
<ul>
<li>Conduct comprehensive psychological evaluations, including diagnostic assessments, mental status exams, and cognitive screenings</li>
<li>Provide individual psychotherapy tailored to the needs of older adults and individuals with complex medical or cognitive conditions</li>
<li>Facilitate family therapy and caregiver support sessions</li>
<li>Lead or co-lead group therapy sessions when appropriate</li>
<li>Provide behavioral management interventions to support residents, facility staff, and families</li>
<li>Develop and implement individualized treatment plans based on evidence-based practices</li>
<li>Manage caseload independently while exercising clinical judgment consistent with ethical and professional standards</li>
</ul>
Interdisciplinary Collaboration
<ul>
<li>Partner daily with nursing staff, social workers, physicians, and rehabilitation teams</li>
<li>Participate in interdisciplinary meetings and care plan reviews</li>
<li>Serve as a behavioral health resource to facility leadership and staff</li>
<li>Offer consultation to facility teams regarding behavioral strategies, resident needs, and care recommendations</li>
</ul>
Documentation &amp; Compliance
<ul>
<li>Complete all documentation in a timely manner using our Geriatric Notes Tool (GNT)</li>
<li>Maintain accurate, comprehensive clinical records aligned with regulatory requirements</li>
<li>Adhere to state laws, CMS expectations, HIPAA standards, and APA ethical guidelines</li>
</ul>
Facility Partnership
<ul>
<li>Foster collaborative and professional relationships with facility personnel</li>
<li>Model professionalism and empathy while representing Deer Oaks</li>
<li>Maintain reliability, punctuality, and strong communication to support consistent care delivery</li>
