Case Manager - We are hiring both locations in Long Island City, NY and Queens. Let us know which location you prefer in the application questions section.The Case Manager&rsquo;s responsibilities are to help the program clients address problems ranging from benefits needs, family issues, health and work issues, and other life events and problems.PLEASE NOTE SCHEDULE &amp; AVAILABILITY FOR THIS ROLE:<ul><li>8:00 AM &ndash; 4:00 PM primary shift</li><li>One late day required per week</li><li>Must be open to working one weekend day</li></ul>Key Responsibilities:<ul><li>Participate in the intake of people for admission to the housing program, by conducting review of documentations and registration of client details.</li><li>Following development of initial assessment, conduct at least semi-annual assessment of residents.</li><li>Develop initial individualized service plan with residents in accordance with the resident's desires, needs, and strengths.</li><li>Review service plan at least quarterly and revise as necessary in accordance with the resident's desires, needs goals and progress.</li><li>Ensure that objectives and services/methods included in the service plan relate to goals in the service plan and relevant for the resident.</li><li>Provide direct assistance to residents in daily life skill areas in which the resident may be deficient and requires skills training.</li><li>Record the progress of residents within required documentation systems and work with other program team members to ensure client&rsquo;s issues are resolved.</li><li>Make referrals of residents to local supportive service providers.</li><li>Respond to resident crisis, emergencies and other situations that require immediate staff involvement and actions.</li><li>Ensure that program operations standards meet program quality standards and compliance with all regulations &ndash; local, state or federal.</li><li>Perform related duties and responsibilities as required by the immediate supervisor.</li></ul>Qualifications: <ul><li>1&ndash;2 years of experience as a Case Manager, preferably in a residential DHS shelter or similar congregate setting required</li><li>Demonstrated experience completing client assessments and developing Independent Living Plans (ILPs) mandatory</li><li>Experience escorting clients to medical, housing, and social service appointments</li><li>Working knowledge and experience of CARES</li><li>Basic housing experience, including familiarity with CityFHEPS vouchers/CURRENT/HOMES and housing navigation support</li><li>Strong organizational, documentation, and time-management skills</li><li>Ability to manage a caseload while meeting compliance and reporting deadlines</li><li>Associate degree preferred (Human Services, Social Work, Psychology, or related field)</li></ul>Job Type: Full-timePay: $50,000.00 per yearPeople with a criminal record are encouraged to applyApplication Question(s):<ul><li>Are you comfortable with the posted salary?</li><li>We are hiring for both the Long Island City and Queens locations; which do you prefer?</li><li>Please confirm you are able to work the required schedule of 8-4pm plus one weekend day.</li><li>Are you comfortable starting as per diem with the potential for this to become permanent?</li></ul>Experience:<ul><li>Case management: 1 year (Required)</li></ul>Work Location: In person
