Schedule:
<ul><li>
Monday - Wednesday - Friday (3 day shifts) 30 hour weekly</li><li>
Day shift only</li><li>
No weekends
</li><li>Fixed schedule/no on-call</li></ul>

Key Responsibilities:
<ul><li>
Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);</li><li>
Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;</li><li>
Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;</li><li>
Develops and maintains a plan of care that describes interventions to attain expected outcomes;</li><li>
Implements and records interventions to attain expected outcomes in EMR;</li><li>
Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;</li><li>
Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);</li><li>
Maintains a positive and respectful attitude to all patients and colleagues;</li><li>
Occasional travel to other nearby locations; and</li><li>
Medication administration and documentation.</li></ul>

Requirements:
<ul><li>Valid RN license</li><li>
BS/AA/diploma in nursing</li><li>
6 months of RN experience strongly preferred</li><li>
3 months of hemodialysis experience strongly preferred</li><li>
BLS certification (American Red Cross or American Heart Association Required); and</li><li>
Ability to lift up to 35 pounds is required</li><li>
This is a safety sensitive position</li></ul>

Compensation:

Salary Range: $40.00 - $60.00 per hour. **BASED ON EXPERIENCE.**

Compensation offers are based on education, experience, skills, and geographic region.
