The Care Partner is responsible for carrying out the Activities of Daily Life services to residents in accordance with their service plan in a given managed community. These services include providing personal care for all residents and communicating with medical professionals and families about the needs of residents under the direction of the Director, Health and Wellness. This position also participates in meal service including serving, clearing, and cleaning the dining room. All services provided by the Care Partner will be conducted in full regulatory compliance.

Responsibilities:<ul><li>
Follows the services outlined in each resident's service plan in a friendly, respectful and supportive manner. Documents any observed change of condition in accordance with applicable laws and company policies.
</li><li>Complies with the Continuous Quality Improvement Program and takes steps to ensure that services delivered are complete and designed for each resident. Reports identified issues not supporting quality services to the immediate supervisor for correction.
</li><li>Understands and follows all company policies as well as all local, state, and federal rules and regulations as they apply to services delivered for all residents, including updates and amendments to those rules, regulations and policies. This involves resident care and documentation, and compliance with HIPAA, OSHA, and Blood Borne Pathogen Guidelines.
</li><li>Works in a safe manner in conjunction with other employees. Reports any unsafe actions immediately to supervisor and ensures such actions are corrected. Compliant with Safety Committee Standards.
</li><li>Fosters teamwork with co-workers, takes actions which assist others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors.
</li><li>Communicates between Care Partners on resident needs.
</li><li>Perform other duties as assigned or needed.</li></ul>
Required Skills and Experience:<ul><li>
High School Diploma or equivalent required.
</li><li>Must have one year of experience in a caregiving role or similar function.
</li><li>Experience with Microsoft Word and Excel preferred.</li><li>
CNA or HHA Preferred
</li><li>Excellent verbal and written communication skills.</li><li>
Able to communicate verbally in person and over the phone.</li></ul>

The Med Tech is responsible for carrying out and overseeing the ADL services to residents in accordance with their service plan in a given community. Responsibilities:
<ul><li>Leads the medication technician team and ensures the services outlined in each resident's service plan are followed in a friendly and supportive manner. This includes passing resident medications in accordance with physician orders and as stated on the Medication Administration Record for all residents.
</li><li>Verifies documentation completion of tasks delivered on a daily basis.
</li><li>Documents observed change in condition in accordance with local rules and regulations and policies.
</li><li>Responsible for communicating with Medication Technician team about resident needs for consistent quality.
</li><li>Participates in meal service including serving, clearing, and cleaning the dining room.
</li><li>Manages staff under their supervision during working hours.
</li><li>Maintain the proper certifications required by local and state licensing agencies which include on-going in-service trainings, accreditations, specific coursework and certifications.
</li><li>Follows and complies with the Continuous Quality Improvement program and take steps to ensure services delivered are complete and designed for each resident. Reports identified issues not supporting quality services to immediate supervisor for correction.</li><li>
Follows all local, state, and federal rules and regulations as they apply to services delivered for all residents. This involves resident care and documentation, compliance with HIPAA, OSHA, and Blood Borne Pathogen Guidelines.</li><li>
Works in a safe manner, supports other employees in working in a safe manner and unsafe actions are corrected. Compliant with Safety Committee Standards. Should workplace injuries occur, when required, assists the Executive Director and Director, Health and Wellness with the investigation, return to work, and management of the injured worker communication with care provider and the Management Company.</li><li>
Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assist others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors.</li><li>

CMA - Certified Medication Aide

The Server is responsible for all activities in the Restaurant and Hospitality Department under the direction of the Director, Restaurant and Hospitality. Responsible for quality of service delivery during meal service, compliance with special diets offered by the community, and communicating with Distinctive Living on all appropriate matters.
Essential Functions
<ul><li>Develops a positive customer service culture in the dining room through staff training. Evaluates the quality of service and develops systems to maximize the efficiency of meal delivery while maintaining a pleasant dining experience.
</li><li>Ensures staff provides service that maintains good resident experiences during meal times. Circulates in the dining room to ascertain resident preferences and satisfaction with meals. Develops service systems that balance quality and speed of meal delivery.</li><li>
Maintains a clean dining room. This includes all equipment, walls, table tops and bases, and chairs. Ensures linens are changed as needed and tabletop condiments and salt and pepper shakers are kept clean.</li><li>
Ensures they and any employees working with them work in a safe manner and that unsafe actions are managed. Compliant with Safety Committee Standard. Should workplace injuries occur, the Department Heads and Directors are to assist the Executive Director when required with the investigation, return to work, and management of the injured worker communication with care provider and Distinctive Living.
