If you are dedicated, proactive, and eager to bring comfort and care to those in need, this is the perfect opportunity for you!Duties<ul><li>Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary </li><li>Monitor patient health by observing vital signs and reporting changes promptly to healthcare providers </li><li>Administer medications accurately following prescribed instructions while maintaining detailed medical documentation </li><li>Support patients with specialized needs including dementia care, Alzheimer&rsquo;s care, memory care, and working with individuals with developmental disabilities or disabilities </li><li>Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and meal preparation </li><li>Use Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions and medical records securely in compliance with HIPAA regulations </li><li>Provide emotional support and companionship to promote mental well-being and reduce feelings of isolation</li></ul>Requirements<ul><li>Proven experience in home health care, nursing homes, assisted living facilities or long-term care environments </li><li>Knowledge of medical terminology and procedures related to patient care </li><li>Valid CPR certification and training in catheter care, hospice care, dementia care, and medication administration preferred </li><li>Ability to work independently while following healthcare plans and protocols precisely </li><li>Strong observational skills for patient safety monitoring including assessing vital signs and recognizing changes in condition </li><li>Experience working with individuals with disabilities or developmental challenges is highly valued </li><li>Familiarity with EMR/EHR systems and medical documentation standards </li><li>Compassionate attitude combined with excellent communication skills and professionalism </li><li>Ability to lift or transfer patients safely using equipment like Hoyer lifts when necessary</li></ul>Join us in providing exceptional home health services that truly make a difference!
