The Personal Assistant is responsible for providing support to program participants with Activities of Daily Living (ADL’s), community involvement and other activities per the written plans of care. This support will be provided while maintaining a safe, stimulating, stable and comfortable setting. Flexible and fun daily atmosphere.
 
Staff are key in providing what is needed to help an individual learn, keep or improve skills necessary to live in a home, providing supports based on an individual's hopes, dreams, and preferences and individuals are afforded privacy and are treated with dignity and respect, to have access to coordinated services and supports determined by the individual's unique strengths, needs and choices. Staff actively participate in all aspects of planning, implementation monitoring and evaluation of member services and supports.
 
MINIMUM JOB QUALIFICATIONS: • Must be able to pass a background and drug test

Personal Assistant

<div>Description:

ESSENTIAL JOB RESPONSIBILITES:<ul><li>
Assesses for immediate and long-term service needs</li><li>
Completion of monthly observation and monitoring.</li><li>
Conducts and Completes Treatment Plans.</li><li>
Assist residents with navigating social service systems and accessing benefits, including Social Security (SSI/SSDI) and other relevant programs.</li><li>
Obtain and maintain SOAR (SSI/SSDI Outreach, Access, and Recovery) certification.</li><li>
Planning, scheduling, and coordinating monthly meetings with residents and stakeholders.</li><li>
Responding to client changes in needs, completing/ submitting appropriate paperwork.</li><li>
Researching, accessing, and linking to services.</li><li>
Communicating and working with providers to ensure appropriate service provision.</li><li>
Meeting with supervisor for weekly supervision and consultation.</li><li>
Attending/Participating in staff meetings with RA&rsquo;s.</li><li>
Assist with formal and informal training of residential staff according to policy and funding source as needed.</li><li>
Monitor the quality of residential programs through established reviews.</li><li>
Complete application for social security and other benefits.</li></ul>
ACCOUNTABILITIES:
I. PROVISION FOR CASE MANAGEMENT SERVICES:
A. Assesses and addresses in ISRP/needs expressed.
B. Make referrals to appropriate agencies and/or providers.
C. Documents and follows up in a timely manner to ensure services are in place and appropriate
D. Establish ongoing rapport with clients and assists them as appropriate in maintaining community living.
E. Responds to client referrals and needs in a timely manner.
F. Empower clients and families to be active participants in their care.
G. Works with the client, service providers and families in setting long term and short term goals for the client; link services to meet these goals; and monitors progress of the client achieving set goals.
H. Follow up on all benefits applied for the residents
II. CLIENT FILE MAINTENANCE
A. Keep accurate and up-to-date records and documentation.
B. Completes Comprehensive Plan of Care and quarterly reviews, amendments and other paperwork.
C. Maintains thorough, up-to-date progress reports. 
III. PROVIDES ASSISTANCE OR FILLS IN FOR OTHER CASE MANAGERS AS NEEDED AND/OR DIRECTED BY SUPERVISOR.
IV. STAFF DEVELOPMENT
A. Work with the Residential Supervisor to ensure maximum communication and coordination with other Easterseals staff, e.g., administrative assistants, other program development staff, etc.
B. Work with Residential Supervisor and RA&rsquo;s in evaluating program effectiveness and compliance within State/Contract requirements and program goals.
C. Participate in training activities and programs as required by funding source, policy, and supervisor.
D. Participate in trainings for certifications, credentials or specialties as determined appropriate by Easterseals Louisiana, funding source or supervisor.
E. Provides input and assistance to Residential Supervisor in the preparation of program reports.

Must have a bachelor&rsquo;s degree in social work or human services-related field and one year experience in the social service field.</li><li>

Behavioral Health Case Manager

<div>Description:

ESSENTIAL JOB RESPONSIBILITIES:<ul><li>
Review Comprehensive Plans of Care (CPOC&rsquo;s) for submission to the regulatory authority</li><li>
Make the initial contact for new linkages</li><li>
Assist support coordinators with Individual Supports Review (ISR) corrections, review revisions, and review Guidelines for Planning State Office Review Committee (GPSORC) forms</li><li>
Review ten percent (10%) of each Support Coordinator&rsquo;s case records monthly for completeness, compliance with these standards, and quality of service delivery</li><li>
Conduct individual face-to-face sessions with each Support Coordinator to review individual cases on a weekly basis</li><li>
Attend and participate in weekly staff meetings</li><li>
Manage assignment of caseloads</li><li>
Accompany Support Coordinators at least annually on random home visits with participants</li><li>
Arrange training as appropriate</li><li>
Direct all assigned staff to meet outcomes</li><li>
Provide ongoing and annual review, assessment problem-solving, and feedback regarding the delivery of support coordination services</li><li>
Teach and monitor the application of person-centered principles and practices</li><li>
Assure ongoing quality delivery of services</li><li>
Review timesheets and mileage reports for accuracy</li><li>
Discipline and coaching of direct reports and annual performance review for each</li></ul>
Requirements:

Qualifications - Support Coordination Supervisors must meet the following qualifications.
A Bachelor&rsquo;s or Master&rsquo;s Degree in one of the following:<ul><li>
Social Work from a program accredited by the Council on Social Work Education AND two (2) years of paid post-degree experience in providing coordination services; or</li><li>
Nursing (RN) currently licensed in Louisiana (one year of paid experience as a licensed RN will substitute for the degree) AND two (2) years of paid post-degree experience in providing coordination services; or</li><li>
Human Service related field (i.e. psychology, education, counseling, social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, and vocational rehabilitation) AND two (2) years of paid post-degree experience in providing support coordination services; or</li><li>
Liberal Arts or General Studies with a concentration of at least 16 hours in one of the following fields (i.e. psychology, education, counseling, Revised 06/2023 social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, and or vocational rehabilitation) AND two (2) years of paid post degree experience providing support coordination services.</li><li>
Must be able to multitask and meet deadlines.</li><li>
Must have excellent written and verbal communication skills.</li><li>
Proficiency in using Microsoft Office Software (i.e. Word, Excel) and computer literacy.</li><li>
Must be a team player.</li><li>
Must complete all training set forth by the State of Louisiana and pass required certification testing.</li></ul></div>

OCDD Support Coordinator Supervisor

<div>Description:

ESSENTIAL JOB RESPONSIBILITIES:<ul><li>
Complete quarterly visits, generating required units, if applicable, observation and monitoring, and monthly contacts</li><li>
Conduct and complete annual CPOC&rsquo;s according to funder&rsquo;s timelines</li><li>
Plan, schedule, and coordinate quarterly meetings with participants and providers.</li><li>
Respond to participant changes in needs, completing/submitting appropriate paperwork.</li><li>
Research, access, and link services</li><li>
Communicate and work with providers to ensure appropriate service provision.</li><li>
Maintain all responsibilities and requirements regarding critical incidents.</li><li>
Meet with supervisor for weekly supervision, consultation, and participant needs.</li><li>
Attend and participate in weekly staff meetings.</li><li>
Attend and complete required training.</li><li>
Support participants throughout emergent situations and all public emergencies including weather-related evacuations.</li><li>
This position is a mandated reporter. The Louisiana Children&rsquo;s Code (Article 610) specifies that mandated reporters shall make reports immediately upon learning of incidents of child abuse or neglect.</li></ul>
Requirements:

Support Coordinator Qualifications:<ul><li>
A Bachelor&rsquo;s or Master&rsquo;s Degree in Social Work from a program accredited by the Council on Social Work Education AND two (2) years of paid post-degree experience in providing coordination services; or</li><li>
A Bachelor&rsquo;s or Master&rsquo;s Degree in Nursing (RN) currently licensed in Louisiana (one year of paid experience as a licensed RN will substitute for the degree) AND two (2) years of paid post-degree experience in providing coordination services;</li><li>
A Bachelor&rsquo;s or Master&rsquo;s Degree in a Human Service related field (i.e. psychology, education, counseling, social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, and vocational rehabilitation) AND two (2) years of paid post-degree experience in providing support coordination services;</li><li>
A Bachelor&rsquo;s Degree in Liberal Arts or General Studies with a concentration of at least 16 hours in one of the following fields (i.e. psychology, education, counseling, social services, sociology, philosophy, family and participant sciences, criminal justice, rehabilitation services, substance abuse treatment, gerontology, and or vocational rehabilitation) AND two (2) years of paid post degree experience providing support coordination services.</li><li>
Reliable transportation so that you may make visits to the field to conduct participant visits. These visits are generally held in the home of the participant. You may anticipate about 50% or more of your time will be in the field on home visits.</li></ul>
