Under the general direction of the Executive Director of the Physician Practices, and for clinical issues only the Provider, CNO, or Physician Practice Nurse, the CMA/LPN will provide direct and indirect patient care in an office setting in accordance assisting in a variety of clinical areas in efforts to meet fluctuating staffing needs during periods of increased workload, staffing shortages, and to help balance off long-term absences (i.e., illness, holidays, maternity leave, etc.), reducing overtime of regularly scheduled staff. Based on a volume change, the incumbent may start their shift in one practice and then be transferred or moved to a different practice to meet operational requirements. The incumbent will communicate with medical staff, other departments, and outside agencies while maintaining confidentiality in accordance with TJC, federal, state, and local guidelines, organizational and departmental policies, and procedures. This position requires self-motivation, creativity, and capabilities to function in a semi-autonomous role within a fast-paced and dynamic environment.
STANDARDS OF PERFORMANCE<ul><li>
Conducted/Transported patients to the exam room and prepared the room and patient for examination.</li><li>
Takes vital signs and records vitals in the chart.</li><li>
Assists the Provider with the care and treatment of patients.</li><li>
Obtains specimens via phlebotomy process and urine specimens by policy/procedure.</li><li>
Administers medications according to established policies and procedures as directed by the Physician or mid-level practitioner.</li><li>
Store/handle/transfer vaccines correctly in accordance with facility policy/procedure and by state, federal, and governing regulations.</li><li>
Administers treatments according to established policies and procedures, Labs, etc., as directed by a Physician or mid-level practitioner.</li><li>
Participates in patient education involving the patient&rsquo;s family when appropriate.</li><li>
Completes the nursing portion of the patient charts in an organized, neat, complete, and accurate manner.</li><li>
Identifies physical symptoms and physical changes and reports to the Physician.</li><li>
Observes and responds to behavioral and physiological changes due to medications.</li><li>
Performs pre-certifications for services requiring authorizations.</li><li>
Prepares patients for ordered procedures: lab, X-ray, surgery, treatment, etc., and completes forms or ordered procedures.</li><li>
Administers IV fluids according to center policy.</li><li>
Returns call to patients.</li><li>
Calls in prescriptions for patients as required.</li><li>
Reviews home care with patients.</li><li>
Communicates with other agencies, physicians, facilities, and pharmacies when appropriate and as directed by the physician.</li><li>
Ensures adherence to proper infection control, OSHA, and safety standards.</li><li>
Provide at least four (4) shifts (8 am to 5 pm) coverages per calendar month.</li><li>
Performs other duties as assigned or required.</li></ul>
Requirements:

Minimum Level of Education: Education level equivalent to high school completion and completion of Licensed Practical Nurse Training course or Registered Nurse.
Formal Training: Management skills with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and verbal and written communications.
Licensure, Certification, Registration: Must have and maintain an unencumbered current Georgia Licensed Practical Nurse or Registered Nurse license, or Medical Assistant Certification, and Basic Life Support CPR certification.
Work Experience: Six months to one year related experience and/or training.</div>

LPN - Full Time

<div>Description:

JOB SUMMARY
Under the general direction of the Practice Manager and the Executive Director of Physician Practice, the Patient Access Representative will provide assistance to non-emergent patients as well as perform the registration for patients presenting for services, maintain documentation, verify insurance, accept payments, in accordance with The Joint Commission, federal, state, and local guidelines, organizational and departmental policies and procedures. Communication with medical staff, other departments, and outside agencies while maintaining confidentiality is required. Position requires self-motivation, creativity, and capabilities to function in a semi-autonomous role within a fast-paced and dynamic environment.
STANDARDS OF PERFORMANCE<ul><li>
Performs registration for all patients presenting for service.</li><li>
Obtains, inputs, and transcribes accurate patient data.</li><li>
Completes necessary forms including proper documentation/signatures, insurance information, either on paper or on scanner.</li><li>
Enters data into the computer with minimal errors.</li><li>
Performs as a cashier for payments and maintains cash receipts with accuracy.</li><li>
Meets non-emergent patients to discuss treatment options and financial options.</li><li>
Assesses insurance status. Verifies insurance benefits. Calculates and collects appropriate deductibles and/or co-insurance from patients on services rendered or to be rendered.</li><li>
Obtain payment of applicable co-payments and/or deductibles according to health plans.</li><li>
Discusses with uninsured patients their financial obligations and referral options as per policy.</li><li>
Will navigate through the Availity or Ability software systems to obtain patient&rsquo;s financial responsibility.</li><li>
Sets up financial agreement according to hospital guidelines for any balance due.</li><li>
Explains Helping Hands program option, if applicable.</li><li>
Scan of all records into electronic medical records.</li><li>
Provides Advance Directives information to all patients as per hospital policy/procedure.</li><li>
Act as an ambassador for the facility by interacting with clients, family members, and staff in a friendly caring manner.</li><li>
Requires completion of Insurance Billing Specialist and Insurance Billing Certification course with Hometown Health.</li><li>
Ensures adherence to proper infection control, OSHA and safety standards.</li><li>
Performs other duties as assigned/needed/required.</li><li>
Provides support for other team members when needed.</li></ul>
Requirements:

Minimum Level of Education: Education level equivalent to completion of High School is required. Completion of a Patient Access or Financial Services-specific to Healthcare curriculum or equivalent is highly preferred.
Formal Training: None Required.
Licensure, Certification, Registration: None Required. Will be required to attain Hometown Health Certification in Financial Counseling by the 90-day review.
Work Experience: Working knowledge of health insurance, deductibles, co-pays, and co-insurance required. Minimum of twenty-four months (2 years) experience in customer service, patient registration, and collections preferred. The job requires a comfort level without of pocket collections activities, as well as a thorough understanding of the accuracy needed for the capture of demographic and third-party payer information.</div>

Patient Access Representative (Medical Office Building)

<div>Description:

JOB SUMMARY
Under the general direction of the DON and Charge Nurse, the Patient Care Tech (PCT) will promote palliative care and comfort for hospital inpatients, swing-bed patients, and hospice care patients, in accordance with Joint Commission, federal, state, and local guidelines, organizational and departmental policies and procedures. PCT gives routine, non-licensed, technical bedside care as requested by licensed staff member as a member of the Nursing Team.
Shift: 7p - 7a
STANDARDS OF PERFORMANCE
1. Provides basic age-appropriate patient care under the supervision of an RN/LPN.
a) Basic hygiene is provided by offering complete or partial bath as needed (i.e. mouth care, hair care, shaving and catheter care) as assistant to the RN/LPN.
b) Vital signs are accurate and documented correctly according to unit specific policies and guidelines and gathered when requested by the RN/LPN.
c) Provides a secondary visualization of patients for pressure areas and promptly reports to RN/LPN.
d) I &amp; O&rsquo;s and weights are accurate and documented according to unit guidelines when requested by the RN.
e) Patient arm bands are checked upon admission, each shift and as needed.
f) Emptying and recording output from foley catheters and/or colostomies.
2. Laboratory policy/procedures are followed.
a) Two patient identifiers are verified prior to specimen collection.
b) Verify ID band with the specimen label.
c) Other specimens: sputum, stools, urine, etc. are promptly delivered or sent to the laboratory as appropriate.
d) Patient armbands are scanned for bedside glucose testing.
1. Collaborates with nursing and respiratory to provide appropriate respiratory care.
a) Encourages turning, coughing and deep breathing for appropriate patients.
b) Removes and reapplies oxygen when instructed by RN/LPN.
2. Demonstrates patient safety awareness.
a) Ensures identification band is on each patient at beginning of shift.
b) Communicates fall risk and activates exit alarm as indicated.
c) Uses proper body mechanics.
d) Turns and positions patients every 2 hours as assistant with RN/LPN.
e) Demonstrates patient safety awareness.
3. Performs tasks or clinical skills as delegated.
a) Maintains awareness of patient&rsquo;s nutritional status and assists with meals as needed.
b) Performs urinary catheter care every shift and as needed.
c) Maintains infection control measures per policy.
d) Assists patients with readiness for discharge ensuring that all patient belongings are gathered. Upon D/C, prepares room for environmental services.
e) Ensures patient rooms are kept tidy and organized.
f) Restock supplies as directed by RN/LPN, to include linen, lab supplies, IV supplies.
4. Maintains customer service as a priority.
a) Customers and co-workers are treated in a courteous manner.
b) New patients are promptly welcomed with patient care items and oriented to unit.
c) Call lights are answered promptly with message communicated to the appropriate person.
d) Listens to shift report.
e) Discusses patients plan for the day at the beginning of shift.

Requirements:

JOB QUALIFICATIONS:
Minimum Level of Education: High School or equivalent; or Patient Care Tech Certification or completion of (1) semester in a clinical nursing program.
Professional Requirements: Graduate of a PCT, Nursing Assistant, EMT, CMA, MOA, or U.S. Military Medic Program or Enrolled in a school of Nursing and has completed one (1) full semester of clinical rotations. Patient Care Tech certification preferred.
Formal Training: Additional training regarding glucose testing. (in-house training provided)
Licensure, Certification, Registration: PCT, C.N.A, CMA, MA, EMT or Paramedic certification with the state of Georgia, or Certified Phlebotomist with demonstrated patient care work experience. BLS Certification required within 30 days of hire.<ul><li>
Substitutions Allowed: Certificate of Completion from the U.S. Military Medic Program or currently enrolled in a nursing program with the completion of one full semester of clinical rotations from an accredited school.</li></ul>
Work Experience: Six (6) months of work experience performing advanced skilled nurse aid duties and responsibilities. Experience performing nonsterile dressing, tube feeding, oral suctioning, tracheostomy care, assisting with catheterizations and blood glucose monitoring, in a health facility such as a hospital or similar medically oriented institution strongly preferred.
