As a<b> Hospital Based Patient Advocate</b>, you play a vital role in guiding uninsured hospital patients through the complex landscape of medical and disability assistance. This <b>onsite, hospital-based role </b>places you at the heart of patient financial advocacy&mdash;meeting individuals face-to-face, right in their hospital rooms, to guide them through the process of identifying eligibility and applying for financial assistance. Your presence and empathy make a real difference during some of life&rsquo;s most vulnerable moments.<br>

<b>Job Summary</b><br>
<br>
The purpose of this position is to connect uninsured hospital patients to programs that will cover their medical expenses. As a Patient Advocate, you will play a critical role in assisting uninsured hospital patients by evaluating their eligibility for various federal, state, and county medical or disability assistance programs through bed-side visits and in-person interactions. Your primary objective will be to guide patients face-to-face through the application process, ensuring thorough completion and follow-up. This role is crucial in ensuring that uninsured patients are promptly identified and assisted, with the goal of meeting our benchmark that 98% of patients are screened at bedside.<br>

<b>Essential Duties and Responsibilities</b></div>
<ul><li>Screen uninsured hospital patients at bedside in an effort to determine if patient is a viable candidate for federal, state, and/or county medical or disability assistance.</li>
<li>Complete the appropriate applications and following through until approved.</li>
<li>Detailed, accurate and timely documentation in both Elevate PFS and hospital systems on all cases worked.</li>
<li>Provide exceptional customer service skills at all times.</li>
<li>Maintain assigned work queue of patient accounts.</li>
<li>Collaborate in person and through verbal/written correspondence with hospital staff, case managers, social workers, financial counselors.</li>
<li>Answer incoming telephone calls, make out-bound calls, and track all paperwork necessary to submit enrollment and renewal for prospective Medicaid patients.</li>
<li>Maintain structured and timely contact with the applicant and responsible government agency, by phone whenever possible or as structured via the daily work queue.</li>
<li>Assist the applicant with gathering any additional reports or records, meeting appointment dates and times and arrange transportation if warranted.</li>
<li>Conduct in-person community visits as needed to acquire documentation.</li>
<li>As per established protocols, inform the client in a timely manner of all approvals and denials of coverage.</li>
<li>Attend ongoing required training to remain informed about current rules and regulations related to governmental programs, and apply updated knowledge when working with patients and cases.</li>
<li>Regular and timely attendance.</li>
<li>Other duties as assigned.</li>

<b>Qualifications and Requirements</b><br>
<br>
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or abilities.</div>
<ul><li>Some college coursework preferred</li>
<li>Prior hospital experience preferred</li>
<li>Adaptability when dealing with constantly changing processes, computer systems and government programs</li>
<li>Professional experience working with state and federal programs</li>
<li>Critical thinking skills</li>
<li>Ability to maneuver throughout the hospital and patients&rsquo; rooms throughout scheduled work shift.</li>
<li>Proficient experience utilizing Microsoft Office Suite with emphasis on Excel and Outlook</li>
<li>Effectively communicate both orally and written, to a variety of individuals</li>
<li>Ability to multitask to meet performance metrics while functioning in a fast-paced environment.</li>
<li>Hospital-Based Patient Advocates are expected to dress in accordance with their respective Client&rsquo;s Dress Code.</li>
<li>Hybrid positions require home internet connections that meet the Company&rsquo;s upload and download speed criteria. Hybrid employees working from home are expected to comply with Elevate&rsquo;s Remote Work Policy, including but not limited to working in a private and dedicated workspace where confidential information can be shared in accordance with HIPAA and PHI requirements.</li>
