<div><div><div>The Clinical Resource Coordinator functions as the clinical nurse manager and oversight Ellis Medical Group Outpatient Practice location(s) (as assigned). Provides support to the Practice Administrator in the planning, coordinating, staffing and supervision of nursing care delivery to ensure efficient and effective patient care. Responsible for compliance with departmental and regulatory standards of care and practice.
<ul><li>In partnership with the patient and family, the CRC collaborates with physicians and other inter-disciplinary team members to establish a plan of care with targeted goals and interventions that lead to optimal outcomes for identified at-risk populations.
</li><li>Works in collaboration with the Practice Administrator, Clinical Educator, and other respective team members to provide care with best practice.</li>
<li>Maximizing the patient&rsquo;s own self-care abilities, the Clinical Resource Coordinator uses the nursing process to assess, plan, implement, and evaluate clinical strategies leading to optimal clinical results, optimal cost utilization of resources, and exceptional patient experience.
</li><li>Utilizes clinical assessment skills to maximize patient access to appropriate care at the appropriate time.</li>
<li>Problem solving, coordination of assignments and schedules, and evaluating of clinical staff performance. CRC serves as the clinical resource for the practice.</li></ul> 

LICENSURE AND CERTIFICATION REQUIREMENTS
<ul><li>Current NYS Registered Nursing license required.</li>
<li>Current BLS required.</li>
<li>AAACN certification preferred.</li>
</ul>
EDUCATION AND EXPERIENCE REQUIREMENTS:
Education: BSN in nursing required. MSN preferred or in progress of completion.
Experience: A minimum of 5 years of clinical nursing that includes experience with nursing assessment, patient education, and nursing care plan development.
<ul><li>Clinical triage experience required.</li>
<li>Leadership experience, 1- 2 years in a clinical setting required; ambulatory care setting preferred.</li>
<li>Strong communication skills</li>
</ul>
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
<ul><li>Excellent analytical and interpersonal communication skills necessary to correspond with patients, families, providers and third party payers.</li>
<li>Demonstrated ability to work collaboratively with physicians and other professionals in a direct and positive manner.</li>
<li>Demonstrated ability to lead a clinical team and mentor non-licensed clinical personnel.</li>
<li>Demonstrated critical thinking skills and ability to prioritize patient care and tasks.</li>
<li>Maintains knowledge of all policies and procedures related to patient care.</li></ul> 

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
 The position is located indoors in a normal office environment. Should be able to push/pull 25 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a key board, writing and filing for extended periods of time. Must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or work station for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language. 

LEADERSHIP RESPONSIBILITIES:
<ul><li>Serves as a clinical resource to all clinical team-members by establishing clinical expertise in management of assigned population. Maintains current standards of care for ambulatory nursing;</li>
<li>Supports the office clinical staff in coordination of the interdisciplinary team;</li>
<li>Supervises daily clinical operations of the clinical staff; works in collaboration with the operations supervisor and administrator to ensure staffing, workflows and processes are in place to support successful operations.</li>
<li>Facilitates regular clinical staff meetings and daily huddles to maintain open lines of communication and awareness of expectations to all clinical team members;</li>
<li>Clinical staff schedule (Coordinate with Practice Manager / Administrator)</li>
<li>Acts as liaison for patients, providers and other care team members;</li>
<li>Partners with office administrator for the evaluation of clinical staff by completing and/or providing Clinical feedback for the annual Performance Evaluations as well as probationary evaluations;</li>
<li>Assists clinical educator and site administrator in development of all clinical workflows that pertain to the practice;</li>
<li>Responds to patients&rsquo; clinical concerns and performs service recovery when needed;</li>
<li>Performs POC testing and manages POC competencies for clinical staff within the practice. The CRC must sign off clinical point of care competency validations and maintain up-to-date records of competencies;</li>
<li>Point of Care daily/weekly/monthly Quality Control/ Verifying data is transmitted/recorded daily (EKG, logs)</li>
<li>Enforce Scope of Practice with clinical staff; ensure clinical staff are working within scope of practice;</li>
<li>Assumes responsibility for education and self-development in collaboration with the Clinical Education and Site Administrator; distribution and review of staff education as related to clinical and regulatory topics;</li>
<li>Joint Commission rounding &ndash; the CRC is responsible for the maintenance of best practices and Joint Commission standards as they apply to the practice(s) to which the CRC is assigned to;</li>
<li>The CRC should performs weekly JC tracers and initiates corrective action for applicable findings; including hand hygiene audits;</li>
<li>Manages sample medications and is responsible for compliance of the sample log and adhering to the sample medication policy (as it applies);</li>
<li>Medications, vaccines and VFC (inventory and ordering)/ Oversee the staff education for VFC;</li>
<li>Anticipates patient equipment and supply needs and delegates responsibility for ordering and delivery of these itemsl</li>
<li>Promotes and reinforces clinical policy and procedure; escalates as needed to practice administrator.</li>
<li>Complete mid-day huddles with providers for pre-visit planning</li>
<li>Works in collaboration with clerical staff for patient scheduling concerns</li>
<li>Able to respond appropriately to clinical emergencies in the office</li>
<li>Delegation and monitoring of clinical tasks/responsibilities;</li>
 <li>Ensures all clinical documentation and correspondence is complete in EMR;</li>
<li>Rooms patients as needed to maintain efficient flow of practice schedule. Back up staff with any needs (rooming, injections etc.)</li>
<li>Other duties as assigned / as required.</li></ul> 

CLINICAL RESPONSIBILITIES
<ul><li>Partners with identified at-risk patients throughout the diagnosis, treatment and follow up to deliver continuity of care.
</li><li>Anticipates the needs of the patient, recognizes and responds to changes in the patient&rsquo;s status and determines priorities of patient care.</li>
<li>Maintains a patient-centered approach to care; Evidence Based Practice</li>
<li>Identifies actual and potential patient barriers to learning. Reports barriers to responsible provider / care team and collaborates with them to develop alternatives to traditional learning resources.</li>
<li>Triages patient symptoms and facilitates the appropriate care</li>
<li>Assesses and manages patients&rsquo; post-operative and post-procedure status and relays treatment plan within the RN&rsquo;s scope of practice</li>
<li>Provides patient education regarding all aspects of their disease process, medications and medical concerns</li>
<li>Monitors pathology and other lab reports; Abnormal values communicated to providers
</li><li>Performing necessary point of care testing</li>
<li>Assumes an active role in the Ambulatory Nursing Quality Committee.</li>
<li>Initiates and obtains prior authorizations for medication coverage by governmental and private insurers as needed.</li>
<li>Focuses on providing each patient with an exceptional service experience.</li>
<li>Rooms patients as needed to maintain efficient flow of practice schedule
</li><li>Responds to clinical pool messages in a timely manner</li>
<li>Refill requests sent to provider; includes looking up I-stop for controlled substances</li>
<li>Other RN responsibilities: Respond to patient Non-triage phone calls and respond to needs; Completing Physician forms and letters (including obtaining any necessary records); prior authorizations; Warfarin/INR monitoring and reporting; Patient swabbing/specimen collection; EKG; patient assessment; Annual Wellness Visits/Medicare physicals; Flu/pneumococcal standing orders; Knowledge of adult and pediatric) vaccine schedules
</li><li>Administration of medications per Ellis policy and appropriate medication rights.</li>
</ul>
