What you'll Do:

<li>Provide high‑quality APRN care across the Suncoast Hospice service area, including consults and advanced care planning.

<li>Perform comprehensive health assessments, including medical histories and physical examinations, to support accurate clinical decision‑making.

<li>Diagnose, treat, and monitor acute and chronic conditions in collaboration with a supervising physician.
</li>
<li>Support patients and families through complex care needs, ensuring comfort, dignity, and alignment with end‑of‑life goals.

What You'll Need:
<ul>
<li>Education: Master&rsquo;s degree in nursing or a related field.</li>
<li>Licensure: Current Florida license as an Advanced Registered Nurse Practitioner, certified in a practice are (if applicable to departmental requirements). Current DEA (Drug Enforcement Agency) license.</li>
<li>Certifications: Valid CPR certification from the American Red Cross or American Heart Association.</li>
<li>NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements).</li>
<li>Minimum of two (2) years of nursing experience is required.</li>
<li>Minimum of two (2) year experience as an APRN with experience in adult, gerontology, or oncology practice preferred.</li>

APRN Suncoast

<div>Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker to join our team.

This full-time position works Monday through Friday, 8:30 a.m. &ndash; 5 p.m, and 1 weekend every six weeks. In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care&mdash;directly enhancing the quality, dignity, and comfort of their hospice experience. The social worker visits our hospice patients in the home, ALF, or SNFs in Belle Glade, Loxahatchee, West Palm Beach, Wellington.

Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You&rsquo;ll Do
<ul>
<li>Provide psychosocial assessment and intervention for patients and families within his or her scope of practice as a member of the interdisciplinary team</li>
<li>Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.</li>

What You&rsquo;ll Need

<li>Degree in Social Work or a related degree such as Psychology. Masters preferred.</li>
<li>One year of experience as a Social Worker. In a hospice setting is preferred.</li>
<li>Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.</li>
<li>Strong written and oral communication skills.</li>
