As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.</p>

<li>Providing excellent care for our patients</li>
<li>Providing excellent customer service to our patients and family members</li>
<li>Collaborate with facility staff to ensure excellent care for you patients</li>
<li>Evaluating new patients, developing plans of care and setting measurable, functional goals</li>
<li>Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes</li>
<li>Supervising Physical Therapist Assistants</li>
<li>Re-evaluating patient progress, revising plans of care and resetting goals</li>
<li>Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.</li></ul></div>

<div><p>Enhance Rehabilitation is currently seeking qualified physical therapy assistant candidates to join our dynamic team. As an Enhance Rehabilitation physical therapist assistant, working under the guidance of a physical therapist, you will be a part of growing team. In this role, you&rsquo;ll assist treating patients by following an individualized plan of care developed by Physical Therapists.</p>
<ul>
<li>Ensure every patient receives quality treatment resulting in excellent functional outcomes.</li>
<li>With assistance of physical therapist, develop and implement physical therapy treatment programs based on each patient's particular needs.</li>
<li>Assist patients with all phases of physical therapy treatment and techniques.</li>
<li>Discuss treatment with patients and record patient conditions and responses to treatments in charts after each visit.</li>
<li>Provide continuous communication with Physical Therapist, Director or Rehab, and facility staff.</li>
<li>Operate and apply knowledge of standard physical therapy equipment.</li></ul></div>
