<div><b>Essential Duties and Responsibilities:</b></div>
<div></div>
<div>Deliver high quality palliative care to patients with medical necessity for palliative service via in person and telehealth options. Assist with design of operational processes associated with development of palliative care program.
<br>
Work with facilities and Eventus employees to ensure the growth of palliative care program. Train and educate internal employees on palliative care principles and processes. Recognize gaps in processes and work with interdepartmental team members to streamline/organize palliative operations.</div>

<div><b>Qualifications/Education:</b></div>
<ul><li>Masters degree in Nursing from an accredited educational institution and a current and valid license to practice medicine issued by the state in which you reside Medical Board. All other certification necessary to perform one&rsquo;s duties must be current and valid, required.</li>
</ul><div>OR</div>
<ul><li>Have graduated from a physician assistant educational program that is accredited by a National Commission on Accreditation of Allied Education Programs.</li>
<li>Have passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.</li>
<li>Licensed by the state in which you reside as a Physician Assistant.</li>
<li>Specialization in geriatric, family, or psychiatry desired.</li>

Palliative Care Nurse Practitioner/Physician Assistant

<div><p><b>Position Summary:
</b></p><p>This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required.</p><br>

<li>Conduct assessments that include the behavioral and social needs of the patients</li>
<li>Meet with families and facility staff to coordinate care as recommended by licensed professionals</li>
<li>Assess the patient&rsquo;s knowledge of their clinical condition and provide education and self-management support</li>
<li>Provide outreach to clients who are having problems with treatment adherence</li>
<li>Assist in the development of the individual goals and behavioral health treatment plan</li>
<li>Coordinate with other agencies and community providers</li>
<li>Maintain timely documentation of work</li>
<li>Serve as an active member of the behavioral health treatment team</li></ul><br>

Education and/or Certifications:</b></p>

<li>Bachelor&rsquo;s degree in Counseling, Social Work, Psychology or related field</li></ul><br>

Skills and Qualifications:</b></p>

<li>Experience in a psychiatric or long term care setting preferred</li>
<li>Customer service experience preferred</li>
<li>Ability and willingness to function independently and proactively in a long term care setting</li>
<li>Effective use of electronic medical records and Microsoft software (teams, excel spreadsheets, etc)</li>
<li>Skilled in written and verbal communication.</li>
<li>Skilled in exercising a high degree of initiative, judgment and discretion.</li>
<li>Ability to handle confidential and sensitive information.</li>
</ul>

This care is provided to adults who reside in skilled nursing, assisted living facilities and private residences through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Psychologists, Licensed Clinical Social Worker, Clinical and Administrative Support Staff.</div>

<div>The Clinician Support Specialist is responsible for enhancing the performance of the clinical care team through supporting and facilitating clinical services with their assigned providers. The Clinician Support Specialist is required to work in the field with the assigned provider as directed by Lead Clinician Support Specialist or Manager/Supervisor. You may be asked to travel to facilities up to 60 miles from your home.</div>

<div></div><div><b>Provider Support</b></div>
<ul><li>Ensure that the clinical provider in the assigned building is able to focus on taking care of their patients for the day</li>
<li>Ensure that each patient is seen as medically necessary per clinical protocol.</li>
<li>Serve as a &ldquo;buffer&rdquo; between provider and families/staff to facilitate proper patient management.</li>
<li>Document appropriate encounter and time for CCM and BHI services</li>
<li>If scheduling is assigned: Scheduling rosters should be updated daily to ensure accuracy and schedules sent timely.</li>
<li>Assist with any day-of changes to the scheduled visits due to acute needs, hospitalizations, admissions, or discharges</li>
<li>Prepare clinical documentation for assigned providers each day</li>
<li>Complete basic opening of encounter note as per discipline protocol</li>
<li>Perform medication reconciliation on all scheduled patients each encounter</li>
<li>Enter appropriate facility chart data on all scheduled patients each day into patient encounter notes and assign to provider</li>
<li>Enter lab and other diagnostic study results into patient encounter and assign to provider as per guidelines</li>
<li>Assist with collection and sorting of all clinical paperwork needed for clinician review and ensure completion each day</li>
<li>Host telehealth sessions for providers as assigned by manager/supervisor, both routine and acute visits</li>
</ul>
<div></div><div><b>Facility Support</b></div>
<ul><li>When scheduling is an expectation for your role:
<ul><li>Share patient schedules with key facility staff as needed.</li>
</ul></li><li>Assist with any triage needs as directed by assigned provider (whether they are in or out of the building that day)</li>
<li>Assist with triage documentation as needed.</li>
<li>Ensure that all needed paperwork is routed for the facility to Eventus providers, office, or families.
</li><li>Assist with consents for any services needed.</li>
<li>Attend stand up meetings to report any needed information from the facility to Eventus providers and vice versa if allowed by facility.</li>
<li>Assist with discharge planning of clinical services as needed.</li>
<li>Ensuring check in and out each day with facility staff regarding patients seen, status, etc.</li>
</ul>
<div></div><div><b>Patient Support</b></div>
<ul><li>Maintain patient confidentiality at all times.</li>
<li>Document appropriate encounter and time for CCM and BHI services</li>
<li>Check all forms of communication several times daily to ensure patient needs are met.</li>
<li>Inform patient and family of recent visits, new orders and defer to provider when needed</li>
<li>Communicate transitions of care to CCM team to enhance patient care and education.</li>
</ul><div>Additional Duties and Responsibilities</div>
<div><ul><li>Clinician Support Specialists are expected to be knowledgeable and adhere to Eventus WholeHealth standards, policies and procedures and outlined in employee handbook including HIPAA rules and regulations.</li>
<li>Clinician Support Specialists should adhere to the Eventus WholeHealth dress code policy i.e. clean, wrinkle-free, matching scrubs with appropriate, closed-toed footwear and name badge.</li>
<li>When duties are completed ahead of scheduled workday, report to your Lead Clinician Support Specialist for any additional assignments.</li>
<li>Clinician Support Specialists should seek further training if uncertain how to proceed with any assignment.</li>
</ul>

<div>High School Diploma or equivalency</div>
<div>Must have a valid driver's license and reliable transportation</div>
<div>Expected to travel up to 1 hour from home address</div>
<div>Certified Clinician Support Specialist degree a plus</div>
<div>Typing speed of 45wpm or greater</div>
<div>Medical Scribing experience preferred</div><br>
