Case manager will help identify appropriate providers and facilities throughout the continuum of services while ensuring that available resources are being used in a timely and cost-effective manner for both the member and the reimbursement source.Case manager duties include:<ul><li>Maintain knowledge of policies and procedures of the state regulated SOURCE manual</li></ul><ul><li>Encourage skill development of Activities of Daily Living through task completion</li></ul><ul><li>Engage in respectful, open communication with individuals receiving services, the responsible party and other family and team members to promote consistent, quality care for members</li></ul><ul><li>Develop effective working relationships with the individuals receiving services, the responsible party and their families while maintaining professional boundaries</li></ul><ul><li>Assist in appropriate self-management involving client, including problem solving, decision making, communication, conflict resolution, anger management, social and recreational skills</li></ul><ul><li>Implement goals and/or plans for clients that focus on improving their health</li></ul><ul><li>Plan, monitor and evaluate actions that meet the member&rsquo;s health and service needs</li></ul><ul><li>Promote quality and cost-effective interventions and outcomes</li></ul><ul><li>Partner with the community to provide and connect service opportunities that help improve the health and well-being of the EDWP population</li></ul><ul><li>Accurately complete case notes, progress notes, document events and produce reports as per the company&rsquo;s policy</li></ul>Qualifications:&middot; Bachelor&rsquo;s degree is required<ul><li>Valid Driver's License</li><li>Reliable Transportation</li></ul>&middot; Basic computer skills (Outlook and Microsoft Word)&middot; LaptopKnowledge of:<ul><li>Case management principles;</li><li>Applicable laws, rules, and regulations;</li><li>Available resources to clients;</li><li>Screening and assessment principles and practices</li><li>Applying applicable laws, rules and regulations</li></ul>Skills:&middot; Outlook&middot; Microsoft Word<ul><li>Willingness to resolve complex and/or sensitive issues</li><li>Ability to prepare and maintain reports and records</li><li>Competence to exchange or convey information</li><li>The ability to work independently, demonstrate effective time management skills and to prioritize effectively</li><li>Aptness professional communication and interpersonal skills</li><li>Prior experience in case management or home health is preferred (will train).</li><li>Acquire the ability to effectively deal with stressful situation professionally</li><li>Maintain a positive working relationship with a wide variety of organizations and individuals</li></ul>Job Types: Contract, Full-timePay: From $30,000.00 per yearEducation:<ul><li>Bachelor's (Preferred)</li></ul>Work Location: Hybrid remote in Kennesaw, GA 30156
