</p><p>Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care.
</p><p>Description of Primary Responsibilities:

<div><ul><li>Will provide examination and will review appropriate laboratory, referral and imaging results to determine a diagnosis.

</li></ul><ul><li>Accountable for meeting clinical care measures.

<div><ul><li>Provide comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages within the context of a family unit in a community setting.
</li></ul><ul><li>Comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.
</li></ul><ul><li>Management of chronic diseases and acute illness with a broad range of knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.
</li></ul><ul><li>Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.
</li></ul><ul><li>Consult with supervising physician or other team members on cases that need further input.
</li></ul><ul><li>Works within and helps to promote the Patient-Centered Health Home model of care.

</p><p>Effective June 1, 2026, all individuals hired into the role must:
</p><div><ul><li>Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge, frequently acquired through one of the following:
<div><ul><li>Completion of an accredited Nurse Practitioner, Certified Nurse Midwife, or Physician Assistant training program, with a minimum cumulative GPA of 2.5; or
</li></ul><ul><li>Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
</li></ul></div></li></ul><ul><li>Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a Master&rsquo;s degree (or other applicable extended training program).
</li></ul></div><p>Technical Skills:
</p><div><ul><li>Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
</li></ul><ul><li>Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
</li></ul><ul><li>Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.
</li></ul></div><p>Licenses &amp; Certifications: Licensed in the state of California as a Nurse Practitioner, Certified Nurse Midwife and/or licensed in the state of California as a Physician Assistant.

 The Medical Assistant &ndash; Front Office is responsible for supporting efficient patient flow and registration duties while performing various aspects of the front office patient visit.

   Responsible for supporting and monitoring patient flow.
  </li>
  <ul>
   <li>
    Scrubs appointments in advance to ensure necessary information is available for patient visit.
   </li>
   <li>
    Assists in the distribution of patients, including walk-ins.
   </li>
   <li>
    Makes appropriate assessment of patient&rsquo;s health needs, including taking temperature, collecting symptoms, etc.
   </li>
   <li>
    Keeps patients informed of wait times and ensures patients are properly flowed in system.
   </li>
   <li>
    Schedules patient appointments.
   </li>
  </ul>
  <li>
   Responsible for performing various aspects of patient visit during check in.
  </li>

    Collects accurate demographics and insurance eligibility information.

    Registers new and established patients according to expected workflows.

    Captures and updates insurance information in the patient&rsquo;s record.

   Ensures collection of nominal fees and patient balances at the time of check in.

    Reconciles and deposits all payments collected at the end of the shift.

    Generates daily bank deposit and necessary reporting to Finance department.

   Responsible for obtaining medical record releases, and ensuring the processing of medical record requests.

    Performs documentation in the patient&rsquo;s electronic health record according to policy and training.
   </li>

   Follows procedures for opening and closing the health center.

   May also be required to perform back office duties

    May also be responsible for maintenance of the mobile health unit.
   </li>
   <li>
    In charge of weekly cleaning and restocking of mobile health unit, dumping septic tank and refueling the vehicle.

    Drives mobile health unit to and from various designated locations (as often as multiple times per day, after-hours, and on weekends).
   </li>
   <li>
    Maintains all vehicle logs and any DMV records as required.
   </li>
   <li>
    Assists disabled passengers and children into and out of vehicle.
   </li>
   <li>
    Immediately reports to supervisor any vehicle safety, maintenance and hazard issues. Cooperate with any 3rd party official or employee assigned to vehicle inspections.

   If an individual has completed a degree at a higher level than required by the role and had a stronger GPA in that program, they may provide proof of GPA from that degree in lieu of the high school diploma.

   Have two years of experience or successful completion of a medical assistant externship with Family HealthCare Network.
