Job Roles
<ul><li>Effectively manage the patient check-in and check-out process from start to finish to include identification verification, update or confirm demographic and insurance information, and ensure appropriate forms are provided and completed accurately.</li><li>
Collect specified co-pays and payments from patients per coverage specifications, and from self-pay patients. Screen for eligibility and financial assistance.</li><li>
Follow policies and procedures in collecting and handling cash.</li><li>
Ensure patient information is placed into the Electronic Health Record accurately and in a timely manner.</li><li>
Confirm, request, and process referrals and authorizations for specialty services.</li><li>
Contact patients to schedule appointments and make reminder/recall notifications.</li><li>
Respond to customer billing and payment inquiries on an as needed basis. May escalate billing issues as needed.</li><li>
Regularly display a proactive approach to customer service by listening to the patient, taking ownership of solutions, and is able to accurately identify customer needs through involvement of leadership in resolving concerns.</li><li>
Perform general clerical duties, i.e., scanning information, tracking data, receiving and sorting correspondence and packages in an accurate and efficient manner.</li><li>

Education/Certifications/Licenses/Registrations
<ul><li>High School diploma or GED equivalent required.</li><li>
Or equivalent combination of education and experience that provide the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.</li><li>
Medical Assistant certification, C-NA certification, or Medical Terminology course preferred.</li><li>
Traveling between sites and other locations is occasionally required. General Job Description
<ul><li>Under general supervision, provides patient engagement services to recruit patients into the program's system of care. Work Environment 
FHCSD's services are all outpatient and designed for those struggling with mild to moderate acuity levels. You will have support from other therapists, case managers, a scheduling team, administrators, and a wide range of other medical professionals in our facilities. The Mental Health Department strives to create an inclusive community that fosters teamwork and continuous learning. In addition to a structured orientation process, all newly hired therapists participate in our Buddy Program so you have a designated peer to help you adjust and succeed in your new position.
Our History
FHCSD is a 501(c)(3) nonprofit organization rich in history and tradition. In 1911, the San Diego Industrial School was built on the site where our first clinic now stands. This school eventually transformed into a settlement house that provided Barrio Logan&rsquo;s immigrant residents with dearly needed social services. When it was announced that the city was closing the building in October 1970, Laura Rodriguez and a group of community members attended a Barrio Logan Community Action Committee meeting where they passionately advocated that the location should continue to function as a community health clinic. When their request was quickly rejected, the group occupied the building in protest.
After eight days of negotiations the City of San Diego and the community leaders reached a resolution. This led to the gifting of the building to the people of Barrio Logan and the establishment of a free clinic. Medical care delivery began two evenings a week, and the health center saw approximately 150 patients each month. This building became our flagship clinic, Logan Heights Family Health Center, and we're proud to honor our heritage by continuing to increase access to quality health care in San Diego. Over the past several decades FHCSD has transformed into one of the 10 largest Federally Qualified Health Centers in the country. We now have over 75 locations, more than 3000 employees, and will continue to grow while we strive to meet the needs of our community.

Lifecycle Ownership (End-to-end)<ul><li>
Procurement Coordination: Work with the leadership team to create/submit purchase requests, validate vendor quotes, track orders, and ensure SKUs, models, warranties, and entitlements are captured correctly.</li><li>
Receiving &amp; Intake: Receive shipments, verify quantities/specs based on purchase order, inspect for damage, and log assets with serial numbers into IT asset management and purchasing systems upon receipt.</li><li>
Asset Tagging &amp; Inventory Control: Affix asset tags, log assets into the IT asset management system, update location and custody records (department, user), and maintain accurate stock levels for hardware and accessories.</li><li>

E-Waste &amp; Disposition: Manage compliant disposal with approved vendors; document certificates of destruction and maintain auditable records (including media sanitization logs).</li></ul>
Software Asset &amp; License Management<ul><li>
Track software entitlements, versions, and allocations; assist with true-ups/renewals.</li><li>
Reclaim licenses from retired devices; prevent over-deployment and ensure compliance with vendors.</li><li>
Maintain application catalogs for standardized imaging/deployment.</li><li>
Data Integrity &amp; Asset Management System Administration</li><li>
Maintain high-quality asset records (serial number, model, warranty, custodian, location, status).</li><li>
Reconcile system data (e.g., AD/MDM discovery as needed) with physical counts.</li><li>
Run scheduled audits and cycle counts; resolve discrepancies and stale records proactively.</li></ul>
Security &amp; Compliance<ul><li>
Follow HIPAA policies for safeguarding PHI; ensure devices meet security baselines (disk encryption, endpoint protection).</li><li>
Perform media sanitization aligned with NIST SP 800-88 standards before redeployment/disposal.</li><li>
Support audit requests from Security, Compliance, and Finance with evidence and reports.</li></ul>

Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women&rsquo;s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.
The breadth of our clinic locations, services and programs has grown over the last five decades, making us the largest community clinic provider of health care to the uninsured in the county and one of the top 10 largest community clinic organizations in the nation. We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.<div><div>
Responsibilities:</div></div><div><ul><li>
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.</li><li>
Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.</li><li>
Creates formalized case plans and goals with clients; clients are high-need and require more intense services. Develops a written care-plan with the client, identifying problems and needs. Plan includes areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals. Updates plan as client's needs change.</li><li>
Provides therapeutic interventions to clients. Provides intensive individual support to high need clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintaining necessary care.</li><li>
Performs other duties as assigned.</li></ul>
Requirements:<ul><li>
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.</li><li>
Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required. Master's degree in relevant field preferred.</li><li>
Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.</li><li>
Traveling between sites and other locations is occasionally required. Must have a valid California driver&rsquo;s license, an automobile, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver&rsquo;s license within ten days of establishing residency. Reasonable accommodation may be provided on a case-by-case basis. Mileage and other reimbursement governed by policy.</li></ul>
