<div><div><p>Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our &ldquo;high touch, high tech&rdquo; clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. We also offer behavioral health and primary care at several local schools in hopes of eliminating barriers to those needing services.</p>
<p>Family Health Services is currently looking for a provider that embodies our core values of innovation, collaboration, adaptability, and integrity to ensure we achieve patient centered excellence. We are recruiting a team-oriented Social Worker or Counselor that would be located in a school-based setting. This person would complete intake assessments, provide individual/family therapy, ensures documentation is completed within specified timeframe in our electronic medical records, coordinate between other departments and agencies, assist in general case management, possess problem-solving skills, and be able to enjoy a fast-paced environment.</p>
<p>This position is full-time, Monday through Friday - no weekends, holidays, or call required!</p>

<div><b>Essential Functions</b></div>
<div>Completes behavioral health diagnostic assessments.<br>
1. Creates positive relationship with patients, their families, guardians or agency representatives.<br>
2. Provides psychotherapy for individuals who qualify for services on site.<br>
3. Advocates and refers individuals with serious mental illness and/or substance abuse/addiction to community agencies for treatment.<br>
4. Networks with local social service agencies and educational agencies to remain knowledgeable about available services and the referral process for those services.<br>
5. Provides crisis intervention services when on site.<br>
6. Frequent contact with staff of community agencies and the general public. Daily contact with agency staff. Must develop and maintain positive relationship with all of the above.<br>
7. When working in the outreach/school district setting, adhere to district policies and procedures for student referrals, release of student information, and receipt of information.<br>
8. Other duties as assigned.<br>
9. Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, other State and Federal Laws). This also includes reporting to the Board of Directors, Compliance Officer, Privacy Officer, supervisor, suggestion box, and any suspected unethical, fraudulent, or unlawful acts of practices.</div>
<div>10. Meet productivity standard of 6 patients a day.</div>
<div></div><div><b>Required Education and Experience</b><br>
1. Master&rsquo;s Degree in Social Work or related field with licensure and credentials per the State of Ohio.<br>
2. At least 2 years work-related experience in social service field.</div>
<div></div><div><b>Preferred Education and Experience</b><br>
1. Experience in a community health setting.<br>
2. Experience working with youth and adult populations.<br>
3. Licensed Independent Social Worker in the state of Ohio.</div>
