<div>Description:

The Residential Case Manager will provide supportive services to men and women in Residential substance use disorder/co-occurring treatment. The case manager works closely with the treatment team to accomplish the mission and service goals of Fellowship House. This position includes the following duties:<ul><li>Room inspections</li><li>Coaching and reinforcement of basic living skills</li><li>Facilitate and lead support groups as needed</li><li>To serve as a liaison between participants and connecting with other community organizations</li></ul>
and resources<ul><li>Work with pretreatment, medium-intensity and low-intensity residential clients in small groups and individually to address basic needs and links to services</li><li>Complete GPRA reporting (Government Performance and Results Act)</li><li>Transportation of clients</li><li>Ongoing alcohol/drug testing of clients.</li><li>Monitoring and support with ongoing medication management, for</li></ul>
specified clients and in support of other case managers<ul><li>Intake, transition, and support of clients in medium and low intensity</li></ul>
residential programs<ul><li>Submit documentation, including billing documentation, in a timely and accurate manner.</li><li>Document all interactions and services, as they occur, to team leaders and</li></ul>
other relevant staff; participate in all staff discussions relevant to client care.<ul><li>Perform similar other duties assigned by the Case Management Coordinator, other onsite coordinator, chief operating officer and/or executive director.</li></ul>
Work schedule specific to this employee:
Holiday coverage in rotation with other staff;
Extended hours when needed to provide continue direct care service
Regular Schedule: Monday-Friday 9:00 AM- 5:00 PM
Requirements:
<ul><li>Bachelor's degree in behavioral health related field preferred, or high school diploma/GED with an ADC or CRSS certification.</li><li>Completion of case management training.</li><li>Valid driver&rsquo;s license with good driving record.</li><li>Must have and annually renew CPR certification, First Aid certification, and Medication Assistance Certification through Fellowship House Nurse Delegation Program.</li><li>Knowledge of all Fellowship House&rsquo;s levels of care, including mission, management, objectives and operations; should continually increase as part of ongoing professional development</li><li>Knowledge of local resources serving the needs of clients (medical, clothing, etc.); should continually increase as part of ongoing professional development</li><li>General knowledge of the bio-psychosocial effects of addiction/co-occurring disorder in the individual, family, and community; should continually increase as part of ongoing professional development</li></ul>
-Awareness of Recovery Oriented Systems of Care - Recovery Principles; knowledge of NIDA Principles of Effective Treatment; should continually increase knowledge as part of ongoing professional development</div>
