Job Description:
First Stop Health is seeking a Part-Time Registered Dietitian, Certified Diabetes Care and Education Specialist who is detail oriented and has a passion for helping patients improve nutrition status, blood glucose control, prevent complications, and enhance overall health. In this role, you will provide expert nutrition counseling, diabetes self-management education, and lifestyle coaching to individuals and groups, collaborate with a multi-disciplinary team, and contribute to clinical operations. You will report directly to the Director of Health Coaching.
The ideal candidate is passionate about advancing the company&rsquo;s Quadruple Aim: enhancing patient care experiences, improving population health outcomes, reducing costs, and bolstering clinician well-being. They are also enthusiastic, service-oriented, curious, and collaborative.
Schedule expectations:<ul><li>
15-20 hours per week</li><li>
2 hours per week must be between 4-8pm CT Mon &ndash; Thu or 8a-12p CT Sat &ndash; Sun</li><li>
Schedule needs to be consistent weekly</li></ul>
Responsibilities:<ul><li>
Support a roster of patients virtually/ telephonically (100% remote from home)</li><li>
Utilize established tools and protocols to assess patient health status and identify health risks.</li><li>
Document goals, behavioral and clinical outcomes, and administrative details of all registered dietitian interactions using the portal.</li><li>
Navigate patients to helpful resources and tools (referrals, health information, etc.) that are made available to you.</li><li>
Conduct comprehensive nutrition assessments, considering medical history, lab values, medications, lifestyle, and cultural preferences.</li><li>
Develop individualized meal plans and nutrition interventions aligned with evidence-based guidelines.</li><li>
Provide diabetes self-management education and support (DSMES), including carbohydrate counting, insulin management, glucose monitoring, and problem-solving skills.</li><li>
Content creation and delivery for customer requests such as webinars and speaking engagements</li><li>
Support implementation of new products/technology</li><li>
Ensure outcome collection and analysis</li><li>
Prioritize whole-person care by collaborating with cross-disciplinary teams</li><li>
Maintain and improve processes by following standards, policies and procedures, and recommending new approaches</li></ul>
Requirements:<ul><li>
Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR).</li><li>
Licensed in your state of residence.</li><li>
Certified Diabetes Care and Education Specialist (CDCES) credential from the Certification Board for Diabetes Care and Education.</li><li>
5+ years of experience preferred.</li><li>
Bachelor&rsquo;s or Master&rsquo;s degree in nutrition, dietetics, or a related field.</li><li>
Strong knowledge of nutrition therapy for type 1, type 2, gestational diabetes, prediabetes, cardiovascular and metabolic health, women&rsquo;s health and nutrition through the lifecycle.</li><li>
Excellent communication, motivational interviewing, and behavior change counseling skills.</li><li>
Fervor for cultural curiosity and experience engaging and working with people across a wide range of social, cultural, and socioeconomic backgrounds</li><li>
Self-starter</li><li>
Flexible and eager to learn new skills</li><li>
Ability to work independently in a deadline-oriented environment</li><li>
Fluency in basic computer skills including Microsoft 365 and EHR systems</li></ul>

In this role, you will provide high quality well-being coaching to help patients positively impact their overall health, collaborate with a multi-disciplinary team, and contribute to clinical operations. You will report directly to the Director of Health Coaching.

Ensure outcome collection and analysis</li><li>
Prioritize whole-person care by collaborating with cross-disciplinary teams</li><li>

Stay informed on coaching best practices, Lifestyle medicine related research, news stories and the competitive landscape</li></ul>
Requirements:<ul><li>
Bachelor&rsquo;s degree and are NBC-HWC certified</li><li>
Preference for 5+ years of experience</li><li>
You will work with patients both face-to-face (15% travel to onsite locations) and virtually/telephonically (85% remote from home).</li><li>
Demonstrate coaching confidence with strong core coaching skills to co-create health related SMART goals and develop and implement effective strategies to reach those goals</li><li>
You have a good understanding of behavior change theories and know how to successfully implement them with individuals and groups.</li><li>
Fervor for cultural curiosity and experience engaging and working with people across a wide range of social, cultural, and socioeconomic backgrounds</li><li>
