 Hourly Pay Rate: $28-$36 Hourly (W-2), $35-$43 Hourly (1099)
 
 
 
 
 Physical Therapy Assistant Job Duties:
 
 
 
 <ul>
 <li>
 Collaborate with daily with PT Supervisor and follow established Plan of Care for all patients.
 </li>
 <li>
 Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson&rsquo;s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
 
 
 
 </li>
 </ul>
 Monitor and document patient progress with the use of innovative EMR software.
 
 
 
 Physical Therapy Assistants employed by Flagstar Rehab will receive:
