<div><h2 class="jobSectionHeader">Essential Job Functions and Responsibilities</h2><ul><li>
Provide direct assistance to agency staff and individuals regarding methods, strategies, and techniques relative to Individual Program Plan implementation.</li><li>
Review individual services (e.g., goals and objectives) for compliance with all standards and state requirements, as they relate to the provision of in-service/staff development.</li><li>
Act as a resource to program service staff. Recommend to the teams the need for further assessment or revision of current methods, strategies, and implementation techniques, etc. Provide adequate follow-up to (e.g., hands on training) in all need areas.</li><li>
Provide coaching and resource expertise for staff to foster skill in implementing ISP programs as well as other staff development issues, not part of the formal ISP process.</li><li>
Will be responsible for providing orientation training to new employees as assigned, including First Aid and CPR instruction. This training entails working evening hours in the group homes to ensure comprehensive follow-up.</li><li>
Work with transportation related issues such as cab bills, changes in city bus routes, changes in charges for use of public transportation, and provides input to the department in determining needs for individualized transportation by Fraser.</li><li>
Act as the coordinator and liaison of Fraser individuals to promote community inclusion and socialization. Provide direct assistance to program services staff to develop/plan events and obtain and share community information.</li><li>
Participate in agency committees as assigned.</li></ul>
PROPERTY<ul><li>
Provides overall supervision and monitoring of the home environment, including coordination of the care and maintenance of the property and working with maintenance staff.</li><li>
Assures completion of monthly documentation within the residences (cleaning lists, menus, fridge temps etc.).</li></ul>
FINANCIAL<ul><li>
Assists people to obtain and maintain eligibility for financial benefits, assists people in managing their personal funds, maintains financial files, and monitors financial status monthly (housing assistance, SNAP, Medicaid, Social Security, etc.).</li><li>
Performs Social Security Representative Payee responsibilities and any additional fiduciary support that people supported may need. This includes but is not limited to meeting all deadlines for reporting or recertification for Veteran&rsquo;s Administration Benefits, Social Security and Supplemental Security, County Benefits, Tribal Monies, Medical Assistance, SNAP, Pre-paid Burials, etc. and the meeting the reporting requirements for these services. It is the responsibility of this position to assure that the people supported have access to the services that they are entitled to.</li><li>
Prepares financial profiles for annual OSP meetings.</li></ul>
PERSONNEL<ul><li>
Works with the Independent Living Services Administrator (ILSA) to interview and hire Direct Support Professionals.</li><li>
Supervises Direct Support Professionals and Residential Coordinators and evaluates performance annually, implementing administrative recommendations.</li><li>
Completes Direct Support Professionals scheduling and hours as assigned and authorizes weekly employee time sheets.</li><li>
Completes census of people supported and reports individuals&rsquo; days absent from facility.</li><li>
Models expertise to the Direct Support Professionals and Residential Coordinators regarding methods, strategies, and techniques relative to the OSP implementation.</li></ul>
PROGRAM<ul><li>
Manages assigned caseload to ensure coordination and completion of QIDP/QDDP responsibilities as they relate to housing, financials, health, and the day-to-day wants and needs of the people supported.</li><li>
Maintains open communication with people supported, guardians and family, Case Manager, Direct Support Professionals, and other team members.</li><li>
Uses their working knowledge of licensing, certification and accreditation standards when observing service delivery to assure that compliance with the standards is being met.</li><li>
Completes and updates General Events Reports, including team member notifications, environmental assessments, and any additional required follow-up.</li><li>
Initiates and documents monthly Guardian and family contacts.</li><li>
Responds to requests from people supported/family/Guardians/vocational and day support providers pertaining to the residence and day to day needs.</li><li>
Assists Fraser, Ltd.&rsquo;s medical services department, as requested.</li><li>
Assures the consistent and accurate entry of program related data by Direct Support Professionals.</li><li>
Participates in internal and external investigations, as requested.</li><li>
Adheres to on-call policy and procedure and participates in the on-call rotation as scheduled.</li></ul>
TRAINING<ul><li>
Coordinates staff meetings and ensures Direct Support Professionals receive both in-service and program specific training.</li><li>
Provides in-home expertise and ongoing modeling for Direct Service Professionals regarding methods, strategies, and techniques relative to OSP implementation.</li><li>
Observes program delivery on an on-going basis to identify additional training opportunities for Direct Support Professionals.</li><li>
Assists with any additional training needs which may arise, as requested.</li><li>
Acquires and maintains CPR &amp; First Aid Instructor Certification, instructing training classes as requested.</li></ul>

Education and/or Experience
MINIMUM<ul><li>
Qualified to act as a Qualified Intellectual Disabilities Professional/QIDP/QDDP (bachelor&rsquo;s degree in special education, learning disabilities, education, social work, or a related field AND one year experience working directly with individuals with intellectual disabilities). Will consider 5 years&rsquo; experience in lieu of degree.</li><li>
Strong interpersonal, oral, and written communication skills</li><li>
Time management skills and ability to organize workload</li><li>
