HERE&rsquo;S WHAT YOU&rsquo;LL DO

Clinical Care &amp; Health Monitoring (40%)
<ul>
<li>Oversee medication administration, set-up, and reordering per physician orders.</li>
<li>Manage medical treatments and monitor residents for changes in condition.</li>
<li>Assess pain and implement both pharmacological and nonpharmacological interventions.</li>
<li>Communicate with physicians regarding changes, test results, and care needs.</li>
<li>Act independently in routine and emergency situations; complete incident reports promptly.</li>
<li>Serve as liaison with physicians and vendors; provide dementia-specific interventions.</li>
</ul>
Care Planning &amp; Documentation (20%)
<ul>
<li>Coordinate post-admission assessments and care conferences.</li>
<li>Maintain accurate documentation in EMR systems; audit charts for compliance.</li>
<li>Participate in quality improvement initiatives and communicate via shift reports.</li>
</ul>
Resident &amp; Family Communication (15%)
<ul>
<li>Communicate effectively with families and physicians.</li>
<li>Respect residents&rsquo; cultural and spiritual preferences; deliver personalized care.</li>
<li>Model Eden Alternative principles and treat residents with dignity and kindness.</li>
</ul>
Compliance &amp; Team Leadership (25%)
<ul>
<li>Assist with admissions, transfers, and discharges.</li>
<li>Ensure regulatory compliance and survey readiness.</li>
<li>Train and oversee Resident Assistants in medication administration.</li>
<li>Support staff development through meetings and continuing education.</li>
</ul>
HERE&rsquo;S WHAT YOU&rsquo;LL NEED
<ul>
<li>Current Colorado LPN or RN license.</li>
<li>First Aid/CPR or BLS certification.</li>
<li>3&ndash;5 years of experience in assisted living or long-term care; dementia care expertise preferred.</li>
<li>Strong organizational, communication, and critical-thinking skills.</li>
<li>Proficiency in EMR systems (PCC experience a plus).</li>
<li>Knowledge of HIPAA, state/federal regulations, and dementia interventions.</li>
</ul>
