 The Registered Nurse &ndash; Clinical Nurse Coordinator is a member of an interdisciplinary care team. This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.
 
 
 
 This position will be 1.0FTE/full-time, five 8 hour shifts Monday-Friday from 8am - 4:30pm. This position will be located at the Overlake Clinic in Bellevue, WA.
 
 
 
 
 
 Responsibilities
 
 
 
 
 Primary Responsibilities:
 
 
 <ul>
 <li>
 Assesses physical, emotional, social, and spiritual needs and evaluates patients&rsquo; adaptation to health changes
 </li>
 <li>
 Develops a plan of care based on patients&rsquo; disease, symptoms, and response to treatment
 </li>
 <li>
 Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management
 </li>
 <li>
 Coordinates with inter-disciplinary team to ensure continuity of care
 </li>
 <li>
 Refers patients, families and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.
 </li>
 <li>
 Implements, evaluates and modifies plan of care
 </li>
 <li>
 Integrates research and evidence based knowledge into clinical practice
 
 
 </li>
 </ul>
 In Addition To Primary Responsibilities Of The Registered Nurses (RN), The Clinical Nurse Coordinator (CNC) Will Have The Following Key Responsibilities:
 
 
 
 Direct Patient Care

 Education patients and families: chemotherapy, central lines, injections, home care
 </li>
 <li>
 Telephone and email symptom triage
 </li>
 <li>
 Dressing changes, medication administration, injections
 </li>
 <li>
 New patient orientation
 </li>
 <li>
 Caregiver support
 </li>
 <li>
 Assessment of needs for additional supportive care resources
 </li>
 <li>
 Prescription refills
 </li>
 <li>
 Result reviews
 
 
 </li>
 </ul>
 
 Indirect Patient Care
 
 
 
 <ul>
 <li>
 Supportive care referrals
 </li>
 <li>
 FMLA paperwork, disability paperwork, Ferry passes
 </li>
 <li>
 Coordination of outside clinical care
 </li>
 <li>
 Ensuring continuity of care with outside providers
 </li>
 <li>
 Collaboration with research team for patients on trial
 
 
 </li>
 </ul>
 
 Communication and Teamwork
 
 
 
 <ul>
 <li>
 Attend regular team meetings with provider and nursing teams
 </li>
 <li>
 Coordination of care with ancillary services
 </li>
 <li>
 Delegation of tasks to appropriate team members
 </li>
 <li>
 Coordinating patient schedules with clinic team
 </li>
 <li>
 Collaboration with infusion RN and other members of the care team to assure patient needs are met
 </li>
 <li>
 Real-time management and escalation of issues in clinic
 
 
 </li>
 </ul>
 
 Professional Development and Clinic Engagement
 
 
 
 <ul>
 <li>
 Document all nursing care activities accurately and in a timely fashion.
 </li>
 <li>
 Demonstrate knowledge and understanding of patient privacy rights.
 </li>
 <li>
 Active participation in continuing education, departmental projects, committee work, and quality improvement initiatives
 
 
 </li>
 </ul>
 
 
 Required:
 
 
 
 
 
 Qualifications
 
 
 
 <ul>
 <li>
 Graduate from an accredited school of nursing
 </li>
 <li>
 Must have current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and must upgrade to an MSL with primary state of residence, WA, within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval.
 </li>
 <li>
 Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two years
 </li>
 <li>
 Additional certification may be required dependent upon department.
 </li>
 <li>
 Critical thinking; the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge
 </li>
 <li>
 Ability to develop clinical judgment.
 </li>
 <li>
 Time Management skills; the ability to organize and manage time and tasks independently

 Ability to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying response.

 BSN prepared nurse
 </li>
 <li>
 OCN (Oncology Certified Nurse) or BMT RN
 </li>
 <li>
 2 years of oncology nursing experience
 </li>
 <li>
 Previous telephone triage experience
 </li>
 <li>
 Previous outpatient clinic experience
 </li>
 <li>
 Takes initiative
 </li>
 <li>
 Good communication skills
 </li>
 <li>
 Skilled at multitasking and organization
 
 
 </li>
 </ul>
 The hourly pay range for this position is from $47.64 to $88.19 and pay offered will be based on experience and qualifications.  Registered Nurses are paid on a step-scale basis and are also eligible for shift differentials and other pay premiums such as advanced degree, preceptor and certification.
