This position offers an exciting opportunity to utilize your diverse clinical skills in a dynamic, patient-centered environment committed to excellence and compassionate care.Duties<ul><li>Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of health status to develop personalized care plans.</li><li>Administer medications, including injections and IV infusions, following strict aseptic techniques and medication protocols.</li><li>Manage complex wound care procedures such as suturing, ostomy care, and dressing changes to promote healing and prevent infection.</li><li>Provide specialized treatments like Botulinum toxin injections for neurological or cosmetic purposes under physician supervision.</li><li>Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and adherence to hospice &amp; palliative medicine standards.</li><li>Educate patients and family members on disease management, medication administration, nutrition, and safety measures within the home environment.</li><li>Document all patient interactions accurately using EMR systems like Epic or Cerner while maintaining HIPAA compliance and medical records integrity.</li></ul>Skills<ul><li>Extensive clinical experience in home health, hospice, or long-term care settings with a focus on patient-centered care.</li><li>Proficiency in electronic health record (EHR) systems such as Homecare homebase, Epic or Cerner for documentation and communication.</li><li>Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.</li><li>Skilled in performing complex procedures including IV insertions, phlebotomy, catheterization, tube feeding management, and sterile processing.</li><li>Critical care experience from ICU or emergency medicine environments enhances ability to manage acute pain and trauma cases effectively.</li><li>Ability to work with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or dementia care needs.</li><li>Familiarity with infection control protocols, laboratory testing procedures, and trauma medicine standards from Level I/II trauma centers or teaching hospitals is highly desirable.
