<div></div><div><b>Job Responsibilities</b></div>
<div><div><div><ul><li>Makes the initial nursing evaluation visit.</li></ul>
<ul><li>Continually evaluates the patients nursing needs.</li></ul>
<ul><li>Initiates and coordinates the plan of treatment and necessary revision. Instructs and supervises the patient&rsquo;s family in the care of the patient and maintenance of a healthy environment for the patient.</li></ul>
<ul><li>Provides those services requiring substantial specialized nursing skill.</li></ul>
<ul><li>Renders nursing care to homebound patients and performs treatments as ordered by the physician.</li></ul>
<ul><li>Assists the patient&rsquo;s family to recognize the daily living problems of the patient through all appropriate services for the patient.</li></ul>
<ul><li>Maintains accurate records of the history of the patient for the supervisor and the Physician especially in reference to drugs, treatments, etc.</li></ul>
<ul><li>Reports to the patient&rsquo;s physician and other appropriate personnel pertinent changes in the patient&rsquo;s condition.</li></ul>
<ul><li>Supervises and teaches all other personnel providing services to the patient.</li></ul>
<ul><li>Participates in the in-service programs attending at least two in-services educational meetings per year and as required.</li></ul>
<ul><li>Participates in nursing reports and follow protocol for agency.</li></ul>
<ul><li>Performs 60-day record review (recertifications)</li></ul>
<ul><li>Participates in staff conference regarding patient care.</li></ul>
<ul><li>Participates in the QAPI plan and process.</li></ul>
</div><div><b>Job Requirements</b></div>
</div><div><div><ul><li>Current licensure in the state of Texas of Compact license.</li></ul>
<ul><li>Preferably has one year experience of nursing care in a hospital or home health setting.</li></ul>
<ul><li>Current driver&rsquo;s license &amp; access to a dependable vehicle with vehicle insurance.</li></ul>
