<div>Responsibilities:<ul><li>
Conducts initial assessment of prospective residents, including off-site visits</li><li>
Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident</li><li>
Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents</li><li>
Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience</li><li>
Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members</li><li>
Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences</li><li>
Coordinates the Community&rsquo;s infection control program while maintaining state regulatory compliance</li><li>
Develop and oversee the Community&rsquo;s medication management program</li><li>
Contributes toward quality assurance and performance improvement initiatives
</li><li>Complies with all rules and regulations regarding confidentiality and privacy of resident information</li></ul>
Qualifications:<ul><li>
Maintains current state nursing licensure. LPN required, RN preferred</li><li>
Minimum of two (2) years nursing supervisory experience</li><li>
Prior senior/supportive living experience strongly preferred</li><li>
Excellent written and verbal communication skills</li><li>
Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members</li><li>
Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity</li></ul>
